items prepared in accordance with Subpart 12.6, and supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. Offers are requested. A written
solicitation will not be issued. In responding to this invitation to
bid, offerors must refer to Solicitation DTCG39-96-B-3KB910. This
solicitation incorporates the provisions and clauses in effect through
Federal Acquisition Circular 90-37. The SIC Code for the services to
be acquired is 8049. The small business size standard is $3.5M. Offers
for the folliwing are due at the CGA Contracting Office by 2:00 p.m.

Registered Dietitian Services, POP 10/1/99 to 9/30/2000, NTE 1040
hours, $------/hour, Item 0005 (Option Year 4), Registered Dietitian
Services, POP 10/1/2000 to 9/30/2001, NTE 1040 hours, $------/hour.
SCOPE-Provide Registered Dietitian Services to include consultation on
food service programs, review of menus, and dietetic services to all
military beneficiaries referred by the medical professional staff at
the U.S. Coast Guard Academy (CGA), New London, CT. Perform services in
accordance with current CGA food services contract, Armed Forces Recipe
Card Service, CG Wellness Instruction (COMDTINST 6100.3), CG Medical
Manual (COMDTINST 6100.1B), CGA Cadet Weight Management Program
(SUPTINST 1020.1), Contact LT A. Yates at (860) 444-8430 to review
these documents. PERSONNEL QUALIFICATIONS-Provide personnel who meet
the following criteria: 1) Possess current state license as required by
Connecticut state law, 2) Possess national registration by the American
Dietetic Association. HEALTH REQUIREMENTS-Prior to commencement of
work, obtain physical examinations for all personnel performing work
and provide a copy of each report to the CGA Clinic Administrator
stating the date physical examination completed and name of physician
who performed examination with a statement regarding the individual's
physical health (a physical examination administered more than 30 days
prior to the commencement of work is not acceptable). The report will
specifically address the presence of contagious diseases including,
e.g., tuberculosis, hepatitis, sexually transmitted diseases, and AIDS.
Personnel with an active contagious disease will not be permitted to
perform work under this contract. PERSONAL APPEARANCE-The Contractor
shall be neat, clean, well-groomed, and in appropriate attire.
''See-through'' material, ''cutoffs'', tank tops, clothing that
displays slogans or pictures are not acceptable attire. Athletic style
shoes may be worn provided they are color coordinated with the
wearer's slacks, skirt, or dress. Facial hair (including beards,
mustaches, sideburns) shall be trimmed so as not to appear unkempt or
unclean or interfere with safe work practices. Clothing shall provide
a professional appearance in keeping with normally accepted community
standards of dress for the work being performed. QUALITY ASSURANCE/RISK
MANAGEMENT-The quality of treatment provided by the Contractor shall
meet or exceed standards of practice as prescribed by the state and/or
federal authority, i.e., American Dietetic Commission. The Contractor
shall indemnify the Government for any liability producing act or
omission by contractor, its employees and/or agents during performance
of dietetic consultative services. The Senior Medical Officer will
evaluate the quality of professional and administrative services of the
contractor periodically by, a) Visual inspection of contractor's daily
activities, b) Review consultations and reports, and c) Health
record-review of Standard Form 600 (health record) entries.
PROFESSIONAL SERVICES-Contractor shall provide services on a routine
part-time basis (20 hours weekly). Normal workday at the CGA is 7:00
a.m. to 3:30 p.m. (Monday-Friday). Services provided at any other time
shall be by prior mutual agreement between Contractor and Contracting
Officer (CO) or Contracting Officer's Technical Representative (COTR,
the Clinic Administrator. Contractor shall provide all aspects of
dietetic consultative services including: 1) Clinical assistance to
include group nutritional counseling, case consultations from referring
medical officer and duty status recommendations, preparation of
documentation for medical staff review and patient's official CG
medical record, periodically follow-up in dining facility observing
meals and noting eating habits of patients, 2) Review and approve food
service contractor menus to assure compliance with current food
service contract. Contact with food service contractor shall be via the
CO, 3) Conduct a minimum of four monthly random inspections each of box
lunches and meals served by food service contractor to ascertain
compliance with food service contract nutritional requirements.
Contractor shall note methods of preparation with view toward
predicting nutritional impact and menu acceptance, 4) Advise Chief,
Health Services and the Food Service Officer regarding special dietary
requirements (e.g., athletic training, inpatient meals), 5) Provide
professional advice to unit Wellness Program Specialist, 6) Provide
technical advice/review regarding development/preparation of new
Statement of Work for food services contract, 7) Teach at least one
clinic training session per quarter regarding review of standard
dietetic topics, new developments, wellness counseling. Coordinate
training with clinic training manager, 8) Serve as ad-hoc member of CGA
Health/Wellness Committee, Clinic Professional Staff Committee, Food
Service Selection Committee, 9) In those cases when Chief, Health
Services Division indicates that an active duty member receiving
consultation services is not Fit For Full Duty (FFFD), the contractor
shall recommend, in writing, an appropriate duty status and duration.
Patient must return to command for final disposition, 10) Contractor
shall refer to Clinic Senior Medical Officer any patient medical
condition that requires evaluation and/or treatment. GOVERNMENT
FURNISHED SUPPLIES & EQUIPMENT-CGA Clinic will provide an office and
supplies commonly utilized as follows (but not limited to): Medical
smock, office supplies, medical forms, booklets, demonstration models,
other disposable supplies. CGA will provide equipment necessary to
perform services as follows (but not limited to): Macintosh 6100 CD or
Standard Work Station III personal computer, Nutritionist III software
program, scales, illustrative charts, body composition machine.
RECORDS, REPORTS, AUDITS- 1) Outpatient Records-Contractor shall only
treat patients who present a CG health or clinical record and refer any
patient seeking care without a clinic record to the medical
administrative staff. Clinic records are sole property of CG.
Contractor is not required or authorized to maintain separate treatment
records, 2) Clinic Records/Record Entries-Each record shall contain the
following information: name, rank/rate or beneficiary category, active
duty member SSN, active duty sponsor unit address/home address (if
patient not active duty member); Entries must be adequate for continued
patient care, to evaluate for quality of care and to substantiate
disability, if indicated. Clinic record entries shall be limited to
either a health record (SF600) or a consultation sheet (SF513). Entries
must be legible and contain at least the following: By the Subjective,
Objective, Assessment Plan (SOAP) for SF600s and as deemed appropriate
by Senior Medical Officer for SF513s, 1) Date of Service, 2) Place of
Service, 3) Chief Complaint, 4) History and Physical Findings, 5)
Diagnostic Procedures (including x-ray), 6) Diagnosis, 7) Treatment
Planning, 8) Duty Status Recommendation (as needed), 9) Medical
Officer's Signature, 10) Contractor's Signature. NOTES: Handwritten
entries must be completed the day services are rendered and Contractor
will have access to necessary medical information for
assessment/counseling. Other forms may be used with Clinic
Administrator approval. All Contractor documentation/record entries
shall be sufficient to justify claims for services presented to CG.
DELIVERABLES-Contractor shall provide following weekly reports, a)
Weekly menus after review/signature, b) Recommended menu changes, c)
Nutritional problems with menus and food preparation, d)
Findings/recommendations regarding meal or box lunch inspections, 3)
Number of patients counselled, general categories of day patients,
counselling conducted (e.g., cadets, active duty, retirees, dependents,
overweight, blook pressure, low salt, etc.). The following provisions
