Q -- MEDICAL SERVICES SOL DMA401-96-R-2618 DUE 061496 POC Ms. Andrea McClendon, Contract Specialist, (301) 227-5130 The Defense Mapping Agency (DMA) intends to procure a Health Care Provider (HCP) for a clinic located at Headquarters (HQ), DMA, 8613 Lee Highway, Fairfax, VA. The HCP shall consist of one (1) Certified Occupational Health Nurse with experience in providing health promotions and health assessments to be present during the hours 0730 through 1600 Tuesday, Wednesday and Thrusday except Federal holidays. In addition to influenza immunizations for HQ a maximum of 150 immunizations will be provided at the Central Imagery Office (CIO) located at 8401 Old Court House Road, Vienna, VA 22182. Additionally, a nurse may be required to provide on-site health promotion services instead of servicing DMA/HQ on those days. The HCP will also make available one (1) licensed Physician for consultation with the HCP nurse and to review laboratory and clinical test results and will be required to perform an annual on-site review of services provided. Services will be for one base year with up to four one year options. All responses must be in writing. All responsible sources may submit an offer which will be considered.
