(405)481-7545. This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. This
solicitation is being issued as Request for Quotation (RFQ)
F34612-96-QV203. The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-36, Defense Acquisition Circular 91-9, and Air Force Acquisition
Circular 92-50. This solicitation is 100% small business set-aside.
Standard industrial classification is 8534 with small business standard
set at $3.5 million. Contract line item is as follows: 0001, Conduct
groundwater and soil investigations around nine removed underground
storage tanks (seven sites), and a past fuel line leak; Quantity: 7
sites. Objective: The purpose of this scope of work is to conduct
additional groundwater (GW) and soil investigations around seven
removed underground storage tanks (UST's) sites, and a past fuel line
leak. Collection of soil and GW samples for analytical testing,
compiling analytical results, and developing reports required by the
Oklahoma Corporation Commission (OCC) must be completed to close the
sites. Background/Current Status: Nine UST's were removed from Altus
AFB, Altus Oklahoma. The former UST sites are located on Altus AFB, and
specific site information can be found in Annex A. All UST's were
removed in spring 1993, and the removal reports are available for
review if requested. Five of the seven UST sites fell under action
level for category II sites. However, the sites weren't all tested for
BTEX and TPH in the groundwater. The contaminated soil was removed and
disposed, and the sites were backfilled. Specific Tasks: The contractor
shall furnish all services, materials, supplies, labor,
superintendence, equipment, and travel required to complete this scope
of work. The Contractor shall conform to all laws and regulations of
the United States and the State of Oklahoma, including all safety and
health regulations and the regulations of the OCC regarding UST's. All
activities shall be supervised by an OCC Certified UST Consultant. A
copy of the Certification shall be provided to the Environmental
Management office prior to starting field work and the Certification
shall remain in effect until the acceptance of the final report by the
OCC. Submit a brief Work Plan that shall include an Accident
Prevention Plan and an Activity Hazard Analysis. The Work plan shall be
approved in writing by the Contracting Officer's Representative (COR)
prior to commencing work. Notify OCC of the pending soil and
groundwater (GW) sampling and satisfy any other OCC notification
requirements. Notify COR of the pending soil and groundwater sampling
in the area of the removed UST's. Soil and GW samples: A Total of 4
borings will be conducted at each removed UST site to a total depth of
30 feet. For each boring, collect soil samples at a depth of 20,25,
and 30 feet deep. Collect one sample of the groundwater in each boring.
All samples collected shall be analyzed for TPH (EPA Protocol 8015
Modified) and BTEX (EPA Protocol 8020). In addition, field screen all
samples with an OVM PID. For laboratory analyses, use an Oklahoma
Department of Environmental Quality (ODEQ) certified laboratory and
approved procedures, including Chain-Of-Custody. An initial copy of the
laboratory analyses shall be sent to the technical manager for review.
The location of all borings, and boring methods to obtain samples
shall be approved by the technical manager for this project prior to
drilling. All borings will be located and plotted by a registered land
surveyor who will use a permanent benchmark as a surveying reference.
Dispose of all soil and water from the borings according to the State
of Oklahoma rules and regulations. Plug and fill each boring according
to the State of Oklahoma rules and regulations. Utilize information
from previous work and results of work conducted in this scope of work
to prepare the following reports for the site: Release Confirmation
Report, Initial Site Characterization and Corrective Action Plan with
an Identification of the soil and GW Remediation Index Category (soil
and GW cleanup levels), Free Product Removal, Final Delineation Report
(a.k.a. Investigations for Soil and GW Cleanup), Risk Assessment,
Remediation Plan (if necessary), and Closure of a Case Report. Submit
all final reports to the OCC. All work plans, lab results, and
draft/final reports shall be sent to 97 CES/CEV. If the analytical
results for any of the GW samples exceed OCC's clean up levels, the
contractor shall provide recommendations on how to proceed with the
site investigation to meet OCC requirement. The required schedule
follows: Submit UST Certification 10 calendar days after award; Submit
Work Plan (WP) 15 calendar days after award; Field Work shall begin 15
calendar days after WP is approved; Submit Laboratory Analytical
Results to 97 CES/CEV as soon as received by the contractor; Submit
Draft Reports 15 calendar days after chemical analyses received by the
contractor; Submit Final Reports 10 calendar days after draft reports
are approved. Annex A contains the following data for the removed
underground storage tanks: Tank number, facility number, size of tank,
contents of the tank, and date tank was removed. Annex A: Tank 298-1,

completed not later than 6 June 96. The provision at FAR 52.212-1,
Instructions to Offerors-Commercial, applies to this acquisition.
Evaluation of award will be based on price. All offerors shall include
a completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certifications-Commercial Items, with its offer.
The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial
Items, applies to this acquisition. The clause at FAR 52.212-5 is
incorporated with the following optional clauses at paragraph (b)
applicable: Paragraphs (1),(6),(7),(8),(9),(10), and (11). The
following optional clauses at FAR 52.212-5 paragraph (c) are
applicable: Paragraphs (1) and (2). Wage Determination 94-2525,
Revision 4 dated 3 January 95 is applicable to all purchase orders
