Contractor shall provide all medical, nursing, and administrative personnel to support a Primary Care Program. Contractor shall serve as the Primary Care Manager for each enrolled beneficiary. Empanelment will be by family at the rate of no more than 2,000 patients per physician. Contract period is 1 July 1996 through 30 June 1997 with four one-year options by the Government to extend services through 30 June 2001.
