510-437-3026. Provide optometry services at the U.S. Coast Guard
clinic located in Petaluma, CA, to beneficiaries referred for treatment
by Coast Guard physicians. This will be a non-personal services
contract and medical liability insurance coverage is required.
Resultant contract will be subject to the provisions of the Service
Contract Act. Contractor will work weekends established by the clinic
administrator, contractor will provide Comprehensive Optometric
Examinations (CPT Code 92004) for an estimated 600 patients and
approximately 150 Brief (established patient) visits, (CPT Code 92499)
during the base period and each option year (an average of 13 patients
per work day). Period of performance will be 01 October 1996 through 30
September 1997 with two, one year option periods. The RFP will be
issued on or about 6 May 1996. This procurement is a 100 percent Small
Business set-aside. The SIC code is 8042 and the small business size
standard is %5.0 million dollars average annual receipts for the
preceding three fiscal years. All responsible sources may submit a bid
which shall be considered by the agency. When calling, be prepared to

(DOT), Office of Small and Disadvantaged Business Utilization (OSDBU),
has programs to assist minority, women-owned and disadvantaged
business enterprises to acquire short-term working capital and bonding
assistance for transportation-related contracts. This is applicable to
any eligible prime or subcontract at any tier. The DOT Bonding
Assistance Program enables firms to apply for bid, performance and
payment bonds up to $1.0 million per contract. The DOT provides an 80%
guaranty on the bond amount to a surety against losses. Loans are also
available under the DOT short term lending program (STLP) at prime
interest rates to provide accounts receivable financing. The maximum
