a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. Solicitation number
DADA13-96-R- 0009 applies and is issued as a Request for Proposals
(RFP). The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 90-37.
The solicitation is issued as unrestricted with a SIC of 3841 and a
size standard of 500 employees. CLIN 0001 Provide one (1) each CO2
Aesthetic Laser, CLIN 0002 Training, to meet the following minimum
technical requirements: 1. Laser shall be capable of being used in
surgical procedures in an outpatient or standard operating room
setting. Laser shall be mobile. Laser shall be CO2 and have the
capability to perform microsurgical cutting ablation and coagulation
with spot sizes at a minimum of .16 mm. 2. Laser shall be adaptable for
use with either Zeiss or Wild operating microscopes and for use with a
Weck Laparoscope. Sight lines when using the microscope shall provide
an unobstructed view of the area. 3. There shall be no discoloration
of tissue associated with the use of the laser. 4. Laser shall be
capable of providing, at minimum, the following pulsed energy levels
with a pulse width of less than 1 millisecond: a. 300 millijoule
setting will deliver at minimum 1-100 watts average power. b. 400
millijoules setting will deliver 2 to 20 watts average power. c. 500
millijoules will deliver 2 to 10 watts average power. 5. The laser
shall provide, at minimum, .5 to 100 watts in the continuous power
mode, with the capability to produce milliwatts when selected by the
operator. 6. Aiming beam shall be a red light and shall produce no more
than 5 milliwatts and track the treatment beam. Treatment beam shall be
delivered along the operator's visual axis. 7. Laser unit shall be
sealed and not require water cooling or other gases. 8. Laser shall
come with two handpieces that will provide a .2 millimeter and 1.0
millimeter spot size, at a minimum. 9. Laser system shall have an
articulating arm that provides the physician with a minimum of a six
foot working radius. Optics within the arm shall be permanently aligned
and shall be sealed so as not to require periodic cleaning. 10. Laser
shall allow for user selection of the appropriate duration and output
power of the laser. The laser shall be equipped with built in safety
mechanisms to limit the output power to therapeutic levels. 11. Laser
shall be no less than five working distances of at minimum 200 mm,
250mm, 300mm, 350 mm, and 400mm. 12. Laser shall meet the requirements
of NFPA 99 for medical devices and shall be capable of using a
hospital supplied 208 volt standard grade receptacle. The Contractor
shall supply a laser warning sign, at least three pairs of goggles for
the correct wavelength of light produced by the laser, and an initial
supply of lens cleaning accessories. 13. Warranty. The Contractor
shall offer the Government at least the same warranty terms, including
offers of extended warranties, offered to the general public in
customary commercial practice. 14. Training shall be provided at no
additional cost to the Government, if it is customary commercial
practice to provide such training. Training class size shall be from
one to three people and shall cover the use and operation of the laser,
any warning or hazards associated with the equipment, and the care and
cleaning of the laser. Informal on-site training shall be provided for
one Biomedical Equipment Repairer to include set up and calibration
techniques required to maintain the laser. 15. The Contractor shall
provide two (2) each operators and maintenance manuals with the laser.
Each operators manual shall include instructions on how to operate the
unit, cleaning instructions, and a listing of supplies and prices for
those supplies that are required to operate the unit. Each maintenance
manual shall include a detailed theory of operation section, parts
breakouts and listing of parts and parts numbers, wiring diagrams
and/or schematics, a list of preventive maintenance procedures and
intervals, complete calibration procedure with recommended listing of
test equipment required, and common troubleshooting problems and
techniques. Delivery shall be F.O.B. Destination, with delivery to be
made not later than 60 days after award of a contract, to Madigan AMC
(Property-MEDCASE), Receiving Section, Bldg. 9040A, Tacoma, WA 98431.
Provisions 52.212-1, Instructions to Offerors-Commercial Items, and
52.212-2, Evaluation Commercial Items apply to this acquisition. The
following factors are included in FAR 52.212-2(a) and will be used to
evaluate offers: Technical capability, Past Performance and Price.
Technical and Past Performance when combined, are significantly more
important than cost or price. Offerors shall submit as part of their
proposal completed copies of the provisions at FAR 52.212-3, Offeror
Representations and Certifications-Commercial Items, and DFARS
252.212-7000, Offeror Representations and Certifications-Commercial
Items, FAR Clause 52.212-4, Contract Terms and Conditions-Commercial
Items, applies to this acquisition and does not include an addendum.
FAR Clause 52.212-5, Contract Terms and Conditions Required to
Implement Statues or Executive Orders-Commercial Items, applies to this
acquisition. The following clauses within 52.212-5(b) are incorporated
in this contract by reference as applying to this acquisition:

52.222-37. DFARS Clause 252.212-7001, Contract Terms and Conditions
Required to Implement Statues or Executive Orders Applicable to Defense

incorporated in this contract by reference as applying to this
