The Federal Correctional Institution (FCI), Sheridan, OR, is soliciting for health services to be provided by a qualified medical entity. The health services shall include the following: inpatient/outpatient hospital services; inpatient/outpatient hospital and physician services will be provided at the contractor's facility(s). Medical services to be provided at FCI Sheridan would include, but are not limited to, the following: optometrist, dermatologist, dietician, radiologist, psychiatrist, etc. Services will be required for a one year base period, with four-one (1) year option periods for a total sixty months from the date of award. Only medical facilities within a twenty-five (25) mile radius from FCI Sheridan will be utilized for inpatient/outpatient services. The government intends to make a single award to the entity which can provide all services. The estimated quantities for each 12 month period of the contract are as follows: base year (inpatient days - 176; outpatient days - 172; option year #1 (194 days and 190 days); option year #2 (213 days and 209 days); option year #3 (234 days and 230 days); and option year #4 (258 days and 262 days). There are approximately 13 specialty medical services to be provided at FCI Sheridan for which quantity and frequency may vary. All responsible sources may submit an offer which will be considered.
