(310)494-5534, Contracting Officer, Sara Trudeau Seeking sources to
provide Customized Admixing, Labeling & delivery of piggyback and
hyperalimentation Products (Ampicillin, Cefotaxine, Nafcillin,
Cefazolin, Pipercillin/Tazobactam, Ceftizoxine, Ceftazidime,
Vancomycin, Amikacin, Clindamycin), Ranitidine, Amino Acid/Dextrose, to
the VA Medical Center, Long Beach. Medications required to admix the
piggyback orders will be supplied by the VAMC, Long Beach. Contractor
must have the capability with the VAMC Long Beach's DHCP Computer
System for transmitting orders directly to the Admixture Service.
Contractor to provide all materials used in preparing the admixture
solutions such as transfer sets, TPN clips, Needless, Syringes, Seals,
swabs, gloves & sharp containers. Piggybacks and hyperalimentation
solutions will be in compliance with the American Society of Hospital
Pharmacists (ASHP) Guidelines & conducted in a Class 100 or Comparable
Clean Room. Contractor shall label & number the I.V. Products with a
label matching those currently @ VAMC, Long Beach. Deliveries to be
made three times daily, seven days a week, within four hours after
receipt or order. Usage records for each product admixed must be
provided monthly. Award to be made for a period of one year from date
of award with the option to award for one additional year.
RFP600-017-96 issues o/A 5/21/96 with proposal due o/A 6/21/96. All
requests for solicitation RFP600-017-96 must be made in writing.
Respondents shall forward descriptive literature substantiating their
