Require the services of a licensed medical physician to work on our site for a maximum of 12 hours per week using our facilities, equipment and supplies. Provide medical services for students ages 16 through 24. Includes exams, diagnosis and treatment of illness and injury, detection and control of communicable diseases, records keeping, effective referrals for special care, participating in medical health education programs. MTC provides nurses and medical assistants. Psychiatric experience is desireable. Must have valid physician license issued by the State of California, and a valid controlled substance registration certificate issued by the US Drug Enforcement Agency. Must have liability and malpractice insurance. This site is a US Department of Labor Job Corps Center operated under contract to MTC, with a student population of 310. Call this center for a copy of the physician subcontract document. All services to be performed in accordance with Job Corps' Policy & Requirements Handbook (PRH), Chapter 6, and Technical Assistance Guide (TAG) #6. The physician will be a subcontractor to MTC. Submit bids as an hourly rate. MTC does not provide any form of benefit package. All responsible physicians may submit a bid which shall be considered by MTC. (120)
