Q -- DENTIST SERVICES TO BE PERFORMED ON OUR SITE, FOR 310 STUDENTS AGES 16 THROUGH 24 SOL HS96-1. Due 052696. Contact Andy Zody, Purchasing Agent, 909-887-6305, ext 205. Require the services of a licensed dentist to work on our site for a maximum of 16 hours per week using our facilities, equipment and supplies. Provide dental services for students ages 16 through 24. Includes exams, routine restorative care, effective referrals for special care, participating in dental health education programs. MTC provides a dental assistant. Must have valid dentist license issued by the State of California, and a valid controlled substance registration certificate issued by the US Drug Enforcement Agency. Must have liability and malpractice insurance. This site is a US Department of Labor Job Corps Center operated under contract to MTC, with a student population of 310. Call this center for a copy of the dentist subcontract document. All services to be performed in accordance with Job Corps' Policy & Requirements Handbook (PRH), Chapter 6, and Technical Assistance Guide (TAG) #6. The dentist will be a subcontractor to MTC. Submit bid as an hourly rate. MTC does not provide any form of benefit package. All responsible dentists may submit a bid which shall be considered by MTC. (120)
