synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. Solicitation DADA16-96-R-0008 herein
is a Request for Proposal. Tripler Army Medical Center (TAMC) has a
sole source requirement for one solicitation line item no. (SLIN), for
maintenance services for Government-owned Acuson equipment, located at
Department of Radiology, TAMC, HI. Equipment to be maintained is as

(Jun 96-30 Sep 96)/1 QT @ $UNIT PRICE = $TOTAL. SLIN 0002: Maintenance
services for above Government-owned Acuson equipment, located at TAMC
(1 Oct 96-30 Sep 97)/4 QT @ $UNIT PRICE = $TOTAL. SLIN 0003:
Maintenance services for above Government-owned Acuson equipment,
located at TAMC (1 Oct 97-30 Sep 98)/4 QT @ $UNIT PRICE = $TOTAL. SLIN
0004: Maintenance services for above Government-owned Acuson
equipment, located at TAMC (1 Oct 98-30 Sep 99)/4 QT @ $UNIT PRICE =
$TOTAL. SLIN 0005: Maintenance services for above Government-owned
Acuson equipment, located at TAMC (1 Oct 99-30 Sep 2000)/4 QT @ $UNIT
PRICE = $TOTAL. The Government intends to negotiate solely with Acuson
Corporation, 2055 Gateway Place, Suite 500, San Jose, CA 95110, for a
sole source firm fixed-price contract. Contractor to furnish labor,
parts, materials, components, tools, transportation and equipment
necessary to provide full service preventative maintenance and repair
for above Government-owned Acuson equipment to be in optimum operating
condition. Preventive Maintenance Services. Contractor shall perform
two (2) scheduled preventive maintenance inspections during 12-month
option period. First inspection shall be performed on or about
effective date of contract and subsequent inspections shall be
scheduled once every six (6) months thereafter. Scheduled preventive
maintenance inspections shall include calibration test and any other
test required to assure instruments are performing to factory
standards. Any worn or defective parts or materials shall be replaced
by Contractor within five (5) work days from date of inspection at no
cost to the Government. Dates of semi-annual preventive maintenance
inspections shall be coordinated in advance with Contracting Officer's
Representative (COR), and will be performed between hours of 7:30 AM
and 4:30 PM, Monday through Friday, excluding Federal Holidays. Repair
Services. For all parts and equipment listed above, Contractor shall
perform unlimited, unscheduled maintenance visits, as required, due to
instrument failure. Maintenance services that are normally performed
on an ''as required'' basis. Included are repairs, modifications,
assembly and installation of equipment. Contractor must respond to
service calls telephonically within one (1) hour. Service calls due to
equipment failure resulting from fair, wear-and-tear will be performed
at no cost to the Government within twenty-four (24) hours after
notification between hours of 7:30 AM and 4:30 PM, Monday through
Friday, excluding Federal Holidays. All repairs requiring replacement
parts shall be completed within five (5) work days. Contractor shall
have a trained, qualified personnel available to respond on-site as
indicated above. Whenever available, TAMC repair technician will
accompany service technician to repair site to observe repairs and
receive appropriate technical service data. Contractor will provide
regional loaner system or peripherals as required during extended down
time. Contractor is required to provide the following documentation
with its synopsis/solicitation: 1) Documentation evidencing technicians
are factory-certified by manufacturer on specific brand and model
specified. 2) Documentation/references evidencing technicians have been
competently servicing applicable medical equipment for at least two (2)
years. 3) Phone no. for a 24 hour hotline for discussing technical
trouble shooting matters and a name and phone no. of designated
individual as POC to respond to telephone calls for service.
Solicitation document, provisions and clauses herein are those in
effect through FAC 90-33. FAR 52.212-1 Instructions to
Offerors-Commercial Items, and 52.212-4 Contract Terms and
Conditions-Commercial Items, applies to this acquisition. Offeror must
include a completed copy of provision at 52.212-3 Offeror
Representations and Certifications-Commercial Items, with its offer.
