Nonpersonal occupational medical/health services for approximately 2,400 employees of Defense Distribution Region West and tenant activities located in Lathrop and Tracy CA to include but not limited to physical exams, audiograms, pulmonary function tests, EKGs, chest x-rays, eye exams, blood lead, zinc protoporphyrin, complete blood count (CBC) with differential, cholinesterase RBC and serum, chemistry panel, urinalysis with microscopic and serum, chemistry panel, urinalysis with microscopic exam, hepatitis A and B vaccinations, carboxyhemoglobin, purified protein derivative (PPD), tetanus and influenza immunization, prerabies vaccination, human immunodefiency virus (HIV) blood test, colon cancer screening (occult blood test), cholesterol screening, health guidance and education, medical surveillance, pregnancy surveillance, maintain medical files on site. Contractor to provide two occupational health nurses (RNs) who will be located at the Government facility - one at the Tracy location and one at the Sharpe (Lathrop) location. OHNs to be under the direct supervision of the Contractor's Occupational Health Physician (OHP) who will be responsible for the Government's occupational medical/health program. OHP will conduct exams off site at contractor's medical facility, review files, monitor employees in the light duty program, review workers compensation cases, make recommendations, attend meetings at the government facility. Hours of operation are 7 am to 3:30 pm, Monday through Friday. Medical facility for exams and testing to be located within a 25 mile radius from the Sharpe (Lathrop) Facility.
