This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with the additional information included in this notice. See note #3, limited to the city limits of Poplar Bluff, MO. The announcement constitutes the only solicitation, bids are being requested and a written solicitation will not be issued. Solicitation 647-13-96 is issued as an invitation to bid (IFB). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 38. The Standard Industrial Classification (SIC) is 8011, Size 3.5. The contract line items (7) are as follows: 1.C/T Scan Without Contrast, estimated qty 120 each. 2. C/T Scan With Ionic Contrast, estimated qty 166 each, 3. C/T scan With Non-Ionic Contrast, estimated 164 each. 4. C/T scan With Guided Biopsy, estimated 2 each. 5. C/T Scan With 3-D View, estimated 2 each. 6. Echocardiogram, estimated 20 each. 7. Mamogram, estimated 80 each. Description of requirements: 1. At the time it is determined a C/T Scan is necessary, MAS will telephone the Contractor and request an appointment. The Contractor must make arrangements to perform emergency scans within four (4) hours of the time of the request. Routine scans must be performed within 24 to 36 hours of the request. The Contractor will read scans, dictate and transcribe results, and forward written report to this facility no later than 4 p.m. on the day following the date of the test. C/T Scan films will be sent to this VAMC for retention at the time the patient is returned to us after the scan. Emergency scans will be read immediately and the results will be called to the referring physician. Written reports will be provided no later than 4 p.m. on the following day in the usual manner and also fax'd to the VA when complete. The C/T Scan will be performed at the Contractor's facility. Ambulance transportation from and to this VA Medical Center for obtaining C/T Scans along with an escort for the VA patient during transportation will be provided by the Contractor at NO charge to this facility or the veteran patient. If a VA patient requires emergency, stabilizing care while enroute or at the Contractor's facility, cost for additional care will be reimbursed by the VA. Prices for C/T Scans will be all inclusive covering the C/T Scan, professional fees, and any medical or pharmacy supplies necessary to perform the test. If the Contractor fails to perform C/T Scan within the required time frame, C/T Scan will be done at the nearest facility with C/T equipment and any additional costs, including the cost of transportation may be billed to the Contractor. Provision 52.212-1 Instructions to Bidders-Commercial, applies to this acquisition. Bids may be submitted on letterhead stationary. Bids must show the following: The solicitation number; time specified in the solicitation for receipt of bids; name, address, telephone number, fax number; a technical description of the items being bid on in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include literature, or other documents, if necessary; a unit price, a total price, and any discount terms for each time period; completed copy of the representations and certifications at FAR 52.212-3 and FAR 52.223-8; acknowledgement of solicitation amendments, statement specifying the extent of agreement with all of the terms, conditions, and provisions included in the solicitation. The bidder agrees to hold the prices in the bid firm for 30 calendar days from the date specified for the receipt of bid. Clause 52.212-4 contract terms and conditions-commercial items applies to this acquisition. Clause 52.212-5 contract terms and conditions required to implement statutes or executive orders-commercial items applies to this acquisition.
