Ford Jr., Contracting Officer 208-378-5100 The Bureau of Reclamation has a requirement to provide professional physician services to enrollees assigned to the Ft. Simcoe Job Corps Center, White Swan, WA. Services are to be performed at the Center. Services to include, but not limited to: initial routine physical exam of corpsmembers, medical history, diagnosis and treatment of illness and/or injury, immunizations, X-rays, referrals to specialists, on-call services, 24-hour emergency coverage, and access to inpatient care. Physicians performing services must be licensed in Washington State. The contractor will be required to perform a minimum of 30% of the services. All requests for this solicitation must be in writing or by fax at (208-378-5108. Telephone requests for this solicitation will not be accepted. All sources responding to this synopsis shall submit information relating to business status, including identification as to whether small, large, minority-owned and/or women-owned. For this acquisition the small business size standard is $3,500,000 annual revenue and the stand and the standard industrial classification code is 8011.
