W -- RENTAL OF MOBILE CT SCANNER SOL 637-48-96 DUE 070996 POC David K. Thomsett, Contracting Officer, 704-299-5830 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 637-48-96 is being issued as a request for proposals (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-36. This acquisition is set aside for small business concerns only. The SIC is 7352. This solicitation is for the lease of one mobile computerized tomography scanner for the 60-day period August 12, 1996, through October 11, 1996, with the option to extend the term of the lease for an additional 30-day period. The scanner will be placed at the VAMC Asheville, NC, facility and shall remain stationary for the duration of the lease period. Scanner system shall have the following minimum specifications: Scanner type shall be Picker Model 1200SX or 1200-Expert with Level II software and Ultra-High speed processing. Format imager will be compatible with Kodak Ektascan HN 35MM x 43MM film, or be an MI10 Matrix Imager. Vendor shall provide electrical receptacle for connection to VA-supplied power conductors. Training/familiarization for VA Radiology technicians shall be provided by the vendor. Vendor shall guarantee a minimum 95 percent up-time. Failure of this provision will result in pro-rata reduction of lease cost for down-time in excess of 5 percent. Mobile coach shall not exceed 45 feet in length and shall include a stretcher lift. Prices submitted shall be all inclusive of delivery, set-up, including connection of receptacle, service, repair parts, X-ray Tube usage, and removal of unit upon completion of lease. System shall be fully self-contained except for electrical power, and shall allow for water, telephone, and computer data line hookup. Delivery of system shall be to: Chief, A&MMS (90), VA Medical Center, 1100 Tunnel Road, Asheville, NC, 28805. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial, without addenda, applies to this acquisition. If other than brand name identified is offered, then the offeror shall indicate the manufacturer, brand, model/part number and a description in sufficient detail to show that the product offered meets the brand name identified. A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items must be included with the offer. The provisions at FAR 52.212-4, Contract Terms and Conditions-Commercial, without addenda, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, without addenda, apply to this acquisition. Companies wishing to respond to this solicitation should provide this office with the following: A price quote on company letterhead showing the solicitation number; separate prices for the sixty-day lease period and optional 30-day extension period; prompt payment terms; correct remittance address; and a completed copy of FAR Clause 52.212-3, Offeror Representations and Certifications-Commercial Items.
