70 -- TELERADIOLOGY SYSTEM SOFTWARE SOL 678-37-96 DUE 062696 POC John Clarno, 520-629-4614 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number 678-37-96 is issued as a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-38. SIC Code is 7379 and the small business size standard is $12.5 million. Contractor shall provide teleradiology system software to include; (1) direct capture transmit station for CT, MR, U/S & NM with 4-channel high-speed direct capture board, transmit software, remote acquisition for modalities. Supports monitor, keyboard and mouse at each workstation. 1 main (CT) and 3 remote (MR, U/S and NM). To include full software documentation, online and written tutorials, installation and training and a 3-year on-site warranty; (2) direct capture transmit station for Angio with 4-channel high speed direct capture board, transmit software. To include full software documentation, on-line and written tutorials, installation and training and 3-year on-site warranty; (3) client software for Windows with full software documentation, on-line and written tutorials, installation and training and a 3-year on-site warranty. Items (1) through (3) above are as provided by CompuRAD, Inc., OR EQUAL. Software is to be capable of transmitting between seven (7) PC's including one laptop portable. Deliver F.O.B. destination 45 days from award date to Chief, Acquisition & Materiel Management Service (90), Warehouse Building 14, 3601 S. 6th Avenue, Tucson, Arizona 85723.
