Furnish all labor, equipment and materials for the repair of five storm damage sites on Forest Service Road 20N04 beginning approximately 42 miles northeast of Ukiah, CA. Major items of work include, but are not limited to, the following: construction staking; claring and grubbing; removal of a small log cribwall; excavation and embankment; construction of three rock lined dips; removal and reinstallation of two culverts with proposed appurtenances; installation of two type 1 inlet assemblies with flume; placement of rock riprap; grading and shaping; and seeding and mulching. To reach the project from Ukiah; take U.S. Highway 101 north for 5 miles to the junction with State Highway 20; proceed east on State Highway 20 for 5 miles to the junction with County Road 240; proceed north on County Road 240 for approximately 10 miles to the old Louisiana Pacific Van Arsdale Mill Site and junction with Forest Service Road M8; proceed east on Forest Service Road M8 for 9 miles to the Eel River Bridge (Forest Service Road M8 becomes Forest Service Road M1 at this point). Continue for 2.2 miles on Forest Service Road M1 to the junction with Forest Service Road 20N04 (Ericson Ridge); proceed northwest 14.2 miles on Forest Service 20N04 to the beginning of the project. Price range is $25,000.00 to $100,000.00. Estimated issue date is 5 July 1996 with an estimated bid opening date of 6 August 1996 at 2:30 p.m., estimated start work date is 16 August.
