Garden Valley Blvd., Roseburg, Oregon, to meet agency needs. Period of performance will include base year plus two additional one year options to extend, beginning approximately September 1, 1996 through August 31, 1997. All requests for RFP must be in writing to the contracting officer or by fax 541/440-1276 and must include name, address, phone number of requestor, along with the solicitation number and reference to Mammograms.
