Q -- MUTUAL USE SHARING AGREEMENT FOR PRIMARY CARE SERVICES SOL RFP #646-1-97 DUE 081596 POC Linda White (412) 365-5436 The Department of Veterans Affairs Medical Center, University Drive, Pittsburgh, PA 15240, is seeking qualified healthcare providers capable of delivering appropriate primary care services to veteran beneficiaries in the St. Clairsville, Ohio geographic area. It is contemplated that a single award will be made to a healthcare facility/clinic qualified to provide primary care services such as, but not limited to, medical history, physical exam, clinical assessment diagnosis, treatment, routine laboratory tests, ECG's, and chest x-rays, to qualified veterans in accordance with contract terms and conditions for the estimated period of October 1, 1996 - September 30, 1997, with two (2) one-year Government renewal options. The area of consideration for the location of the facility is limited to the corporate city limits of St. Clairsville, Ohio. The Government estimates the workload to be 1,500 unique veteran patients for the Base Year with each individual veteran being seen an average of three (3) times for a projected total of 4,500 primary care visits. The offeror will quote a capitation rate for the Base Year and each Option Year. The estimated issuance date for this solicitation is July 16, 1996, and the estimated due date for proposals is August 15, 1996 (C.O.B.).
