65 -- HOME OXYGEN SOL RFP 521-012-97(TP) DUE 082296 POC Teresa Posey Provide home delivery of oxygen to beneficiaries of the DVAMC's Birmingham and Tuscaloosa for the period of 10-1-96 through 9-30-97 with four one year renewal options. The area of service will be the entire state of Alabama with a few exceptions to extend 0-50 miles beyond the state lines. There will be approximately 300 patients. The contractor must be JCAHO certified. The contractor will be required to set-up all new prescriptions within 24 hours of initial notification. Response time to patient calls when there is an equipment or oxygen supply problem shall be within 24 hours. The 24 hour a day 7 day a week response time is inclusive of weekends and holidays. Proposals submitted will be evaluated in descending order of importance. A) Company Experience and Demonstrated Capability, B) Quality Assurance Plan, C) Personnel Qualifications and D) Price.
