The Federal Bureau of Prisons intends to award a service contract for Non-Personal Service as an Audiologist. Duties to be performed at the Federal Correctional Institution, Old Highway Road 75, Butner NC 27509-1000. Services include, but are not limited to performing audiometric exams and hearing consultations on inmates and staff that are referred to the contractor by FCI Butner medical staff members, as well as all in patients. Requirements include certification in the field of Audiology and hold a current State license that permits practice in this professional specialty in the State of North Carolina. This contract will include a base year not to exceed (12) twelve months from date of award, with four (4), one (1) year option periods for renewal. Estimated number of sessions for the base year is 48 (four (4) session per month, one (1) session equals two (2) hours). Option year #1 estimated sessions is 48; Option year #2 estimated sessions is 48; Option year #3 estimated sessions is 48; Option year #4 estimated sessions is 48. The anticipated issue date of the Request for Proposal is July 22, 1996 with an anticipated receipt of offer due by August 12, 1996.
