Courier services will be provided by contractor. Contractor's laboratory must meet the following requirements for accreditation: 1) accredited by the College of American Pathologists, a state or regional reference laboratory, or licensed under the Clinical Laboratory Improvement Act of 1967. 2) Shall successfully participate in the College of American Pathologists comprehensive survey program. 3) Shall have a Federal Identification Number isssued by the Center for Disease Control. 4) Any approved subcontractors utilized must meet the above requirements. A remote one-way teleprinter terminal shall be provided by the contractor. All services shall be rendered or supervised by individuals fully qualified and licensed as required by law. Period of performance shall commence on about 27 December 1996, and will include a base period and three option years. This procurement is unrestricted; all responsible sources may submit an offer.
