Thomas, 706-731-7176 Furnish eyeglasses for Optometry, Ophthalmology and Blind Rehabilitation Clinics, VA Medical Center, Augusta, Georgia, at an estimated quantity of 1500 pairs (approximately 300 pairs to be safety glasses) for a one-year period. Contractor shall furnish all necessary labor, tools, equipment and materials to provide prescription eyeglasses, lenses, frames and accessories. Delivery is required within 5 days after receipt of prescription. The contract period will be October 1, 1996 through September 30, 1997, with two (2) one-year options to extend.
