Provide Dental Hygiene Services for the U.S. Coast Guard Air Station Clinic, Cape Cod, MA on a routine, part-time basis, Monday through Thursday seven and half (7-1/2) hours per day and Fridays four (4) hours per day, to all beneficiaries referred by the professional staff of the Air Station Clinic. All services shall be performed in accordance with the Statement of Work and all other terms and conditions of the solicitation. This service will include professional services, instruction and supervision. All interested offerors shall possess and maintain for the duration of the contract, a current unrestricted license for the practice of Dental Hygiene in the state of Massachusetts. Contract performance period from 01 October 1996, or date of award whichever occurs later, through 30 September 1997 and four (4) one-year options. This solicitation is 100% small business set-aside. The SIC Code is 8011 with $5 million or less annual receipts for small business within the last three (3) fiscal years. This solicitation will be issued on or about July 30, 1996.
