The work may be performed in any of the states in our region (Delaware, Maryland, New Jersey, New York, Pennsylvania, Virginia, Washington, D.C. and West Virginia). Interested vendors who would like to receive a ''Contractor Screening Information Request'', form should send a written request to the Federal Aviation Administration, Fitzgerald Federal Bldg. #111, JFK International Airport, Jamaica, New York, 11430, Attn: CSIR, AEA-55A, or FAX request to (718) 995-5685. No telephone requests will be accepted. To make this initial list, all forms must be obtained, filled out completely, and returned to this office by August 30, 1996. Only those vendors who qualify will be considered for future contracts. All forms submitted after August 30, 1996, will be evaluated. Qualified vendors will be added to the list on a quarterly basis.
