Despres (210)221-9174 (Site Code DADA10) This is a COMBINATION SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR subpart 12.6 as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number is DADA10-96-R-0089 and is issued as a Request for Proposal (RFP). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 90-38. This is a Sole Source procurement. The Standard Industrial Classification (SIC) is 3845, Size 500 employees. DESCRIPTION: The Government intends to procure a Coherent Ultrapulse Carbon Dioxide Laser 5000C, 1996 Model, to include a Smoke Evacuator and Accessory Kit which includes footswitch, ULPA filter, tubing (Non Sterile), fluid canister ring, and laser resistant wand from Coherent, 3270 Bayshore Road, Palo Alto, CA 94303. Delivery is to be made to Darnall Army Community Hospital, Fort Hood, TX 76544 within 60 days after contract award. Acceptance and Inspection shall be at Darnall Army Community Hospital, Fort Hood, TX. FOB Fort Hood, TX. SALIENT CHARACTERISTICS: Unit shall be capable of providing aesthetic laser surgery to be used for plastic surgery, dermatology, oral surgery, and ear, nose and throat (ENT) procedures. It shall have a controlled scanning unit to allow pulsed laser beams to be spread over a pattern of various types of shapes that can cover a wide surface areas of skin. Laser shall be capable of generating 200 pulses per second, and shall provide energy in a collimated pattern with a collimated beam to allow consistent and reproducible results. It shall be transportable. On-site training is required for all operators and medical equipment repairers at a time mutually agreed to by the Government and contractor. The best commercial warranty is requested and shall be identified in offeror's proposal. Offeror shall provide 2 copies each of the user/operator manual, user maintenance/service manuals, warranty, and wiring schematics upon delivery of the laser. SUBMISSION OF OFFERS: Although this synopsis references Note 22 in accordance with FAR 5.207(d)(3), this notice will serve as the combined synopsis/solicitation to be issued to the sole source identified above. Any offers received in response to this synopsis/solicitation shall be evaluated to determine if alternate sources exist and if competitive proposals should be requested. If alternate sources are identified, competitive proposals will be requested at a later date. Otherwise, the government intends to negotiate with Coherent, Inc. and award a contract as a result of this synopsis/solicitation. If an interested party believes they can meet the above specifications, they must submit proposals on letterhead stationary, and must show the following: Solicitation number, a price quote, FOB, prompt payment terms, the manufacturer, brand, model/part number and a description in sufficient detail to show that the product offered meets the brand name identified. FAR provision 52.212-1, Instructions to Offerors - Commercial Items is included by reference. The following are addenda to this provision: At paragraph (c) period for acceptance of offers, the offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers. At paragraph (e) multiple offers have been determined by the contracting officer to be not applicable to this solicitation and is removed. At paragraph (h) reference to multiple awards is not applicable to this requirement. A completed copy of the provision at FAR 52.212-3, Offers Representations and Certifications-Commercial Items must be included with offer. The provisions at FAR 52.212-4, Contract Terms and Conditions Required to Implement Statutes or Executives Orders-Commercial Items, without addenda, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executives Orders-Commercial Items, without addenda apply to this acquisition.
