This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a simplified acquisition procurement. The solicitation number is N0016897RC02L12 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-37. This procurement is set aside for small business manufacturers. The Standard Industrial Code (SIC) for this acquisition is 3841. Offerors shall provide a description, manufacturer's brand, model number, and part number of the product in detail to ensure compliance with the requirements of the solicitation. Description of items to be acquired: CLIN 0001: Standing Order for sterile disposable cadiac cath packs for the period 01 Oct 1996 through 30 Sep 1997. Delivery would be for 50 packs the first and third week of each month. Total quantity required is 1200. NOTE: Supplier must be able to adjust the components of the the pack based on the requirements of the Cardiac Cath Laboratory during the delivery period. Preview of the prospective pack by the Cardiac Cath Lab is required prior to award.
