synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. Solicitation Number 578-39-96 is
issued as a Request For Proposal. This solicitation document,
incorporated provisions and clauses are those in effect through FAC
90-37. Hines Veterans Administration Hospital (VAH), Hines, Illinois,
60141, intends to purchase One Each MLA (Medical Laboratory Automation,
Inc.) AT2000 Patient Transfer system with Semirigid, pliable Teflon
coated transfer board and handset or equal. System enables one nurse to
safely transfer patients up to 500 pounds, with the push of a button.
System includes Digital Patient Scale w/Lighted display. Patient Side
Rails, End Hand Rails and handles for transfer. Safety Belts (2),
Footman Loops (4 sets) and Caster Wheels. Transfer surface consists of
electrically conductive upper and lower transfer belts, upper and
lower separator assemblies. I.V. Pole Mounting (4) and Storage for IV
Poles and Oxygen bottles. Standard Mobilizer Switch Panel contains
on/off switch, tilt controls, and indicators. Delivery is to be made
FOB Destination within 30 days of contract award to A&MMS, Warehouse,
Bldg. #220, Hines, Illinois, 60141. The provision at 52.212-1
Instructions to Offerors - Commercial, applies to this acquisition with
no addenda. In accordance with the provision at 52.212-2 Evaluation -
Commercial Items. The Government will award a contract resulting from
this solicitation to the responsible offeror whose offer conforming to
the solicitation will be most advantageous to the Government, price
and other factors considered. The following factors, in a descending
order of importance, will be used to evaluate offers: (1) Semirigid and
pliable teflon coated transfer board, (2) Handset - allows the operator
to perform all transfer functions.(3) Digital Patient Scale - Weighs
Patient in kilograms/pounds- appears on lighted digital display panel,
(4) Battery operated with A.C. connectors for recharging (5) Warranty.
All offers are to include a completed copy of the provision at
52.212-3, Offerors Representations and Certifications --Commercial
Items, with offer. The clause at 52.212-4, Contract Terms and
Conditions applies to this acquisition with no addenda. The clause at
52.212-5, Contract Terms and Conditions Required to Implement Statues
or Executive Orders--Commercial Items, applies to this acquisition with
none of the additional FAR clauses cited in the clause applicable to
this acquisition. Offers are to be submitted on August 16, 1996, by
close of business (4:00 PM Central Standard Time) to Contracting
