Officer (90C4) This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. Solicitation
number 578-35-96 is issued as a Request For Proposal (RFP). This
solicitation document, incorporated provisions and clauses are those in
effect through Federal Acquisition Circular 90-39. Hines Veterans
Affairs Hospital (VAH) intends to purchase one each Apollo ISM/CDM for
Windows (Host Site) Telepathology Link System,or equal, item number
WIMSCDM-H-G, from Apollo Software Inc. Purchase to include one each
Maintenance and Software Agreement, or equal, item number MS-Q. This
procurement is a sole source, follow-on contract, for accessory
equipment necessary to extend a telepathology link to the Hines VAH
geographic area from an already fully implemented Apollo telepathology
link system between the Milwaukee VAH and Iron Mountain VAH geographic
area. Equivalent system must provide a fully intergrated remote
controlled telepathology link system which produces images compatible
with Veterans Administration (VA) Decentralized Hospital Computer
Program (DHCP). System must allow a seamless remote control environment
for the processing of pathology images for consultative and diagnostic
purposes, system must provide connectivity to the current Wide Area
Network (WAN) utilized within the VA. This connectivity must allow for
the dynamic control of microscopes (control of all stages),
transmission of real time,color, images up to 1k x 1k x 24 bit and
transmission of all other pertinent data and/or video necessary for
pathological consultations and patient diagnosis between Milwaukee,
Iron Mountain and Hines VAH geographical regions. System must be
completely compatible and correlatable to existing Apollo Telepathology
Link System impemented in the Milwaukee and Iron Mountain VAH region
and operate in a Microsoft Windows environment. Delivery is to be
F.O.B. destination within 30 days of contract award to Hines VAH Supply
Warehouse, Bldg. 220, Hines, Illinois 60141. The provision at 52.212-1,
Instructions to Offerors --Commercial Items, applies to this
acquisition with no addenda. In accordance with the provision at
52.212-2, Evaluation--Commercial Items, the Government will award a
contract resulting from this solicitation to the responsible offeror
whose offer, conforming to the solicitation, will be the most
advantageous to the Government, price and other factors considered. The
following factors, in a descending order of importance, will be used to
evaluate offers of equivalent instrumentation: (1) Compatibility and
correlatability to existing VA owned Apollo Telepathology Link System
and VA DHCP system, (2) Ability to provide fully intergrated,
clinically proven, telepathology system, with connectivity to VA
utilized WAN, that provides a seamless remote controlled environment
for the processing and transmission of pathology images as per the
specifications, (3) Price of system (4) Warranty. Offerors of
equivalent equipment are to include a completed copy of the provisions
at 52.212-3, Offerors representations and Certifications--Commercial
Items, with offer. The clause at 52.212-4, Contract Terms and
Conditions--Commercial Items, applies to this acquisition with no
addenda. The clause at 52.212-5, Contract Terms and Conditions Required
to Implement Statues or Executive Orders--Commercial Items, applies to
this acquisition with none of the additional FAR clauses cited in the
clause applicable to this acquisition. Offers of equivalent equipment
are to be submitted on August 15, 1996, by Close of Business (4:00 PM
Central Standard Time) to Contracting Officer (90C4), Edward Hines Jr.
