Christine Albin, Purchasing Agent, (301) 402-3355. The National
Institute of Health intends to sole source with Aravind Hospital, 1
Anna Nagar, Madurai, India 625 020 for the following services. The
Contractor shall develop and deliver a standardized evaluation protocol
in accordance with the Statement of Work/Specifications herein. The
period of performance shall be Base Contract Period: through (to be
completed at time of award). Standardized Evaluation Protocol: Shall be
as provided by Aravind Eye Hospital, 1 Anna Nagar, Madurai, India 625
020, or equal. Shall include the workshop, protocol, data forms,
analysis software and operations manual. Shall be in accordance with
the Statement of Work/Specifications herein. During this phase, the
scope of protocol shall be defined. The evaluation shall cover
indicators that shall help measure the impact of the program in the
community and on the individual. Thus, the evaluation shall track
changes in disease prevalence and the impact on the quality of life of
operated patients. The protocol shall also address the quality of eye
surgery and visual outcomes. C.3.2 Phase II - (Development) Based on
the information gathered in Phase I, a detailed protocol, data
gathering forms, operations manual,a nd an analysis plan shall be
prepared. During this developmental process, no less than two (2) pilot
studies shall be conducted to check the validity of the sub-sections of
the protocol and data forms. In addition, software for data entry and
data management shall be developed. The protocol shall also include
quality assurance measures in data collection. The development of the
protocol shall be an iterative process of development, testing and
refinement until a satisfactory protocol is developed for field testing
and subsequent use. C.4 DELIVERABLES The Contractor shall provide
deliverables in accordance with the following schedule: ITEM QTY

Terrence Gillen 16 months The Aravind Eye Hospital in Madurai, India
has been identified as the only known source available that has the
expertise to perform the work required for the development of this
standardized protocol. Therefore, it is the Government's belief that
these requirements can only be met by the Aravind Eye Hospital.
Concerns that respond to this notice will be considered only if
responding with clear and convincing documentation demonstrating their
ability to meet all the requirements of this synopsis. Written
responses should contain sufficient technical information to permit
agency analysis to establish a bona fide capability to meet the stated
requirements. Adequate detailed data by which to evaluate capability
