This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a simplified acquisition procurement (SAP).The solicitation number is N00168-97-Q-L016 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-37. This procurement is set aside for small business manufacturers. The Standard Industrial Code (SIC) for this acquisition is 2835. The small business size standard is 500 employees. Offerors shall provide a description, manufacturer's brand and part number of the product in detail to ensure compliance with the requirements of solicitation. Standing Order for a base period of one (1) year (Lot 1 Base Year) effective 1 October 1996 through 30 September 1997 with two (2) year options. Lot II - Option 1, period of performance 1 October 1997 through 30 September 1998. Lot III - Option II, period of performance 1 October 1998 through 30 September 1999. Description of items to be procured as follows: Manufacturer Immucor Inc. (1) Fifty-two (52) packages #5012 CAP P Ready Screen 2 test kits/pkg, two (2) packages to be delivered every two weeks; (2) Twenty-six (26) packages #5047 CAP R Ready Screen I and II 480 test/10 plate, one (1) package to be delivered every to weeks; (3) Twenty-six (26) packages #5057 CAP R Ready Screen Pooled 960 test/10 plate, one (1) package to be delivered every two weeks; (4) Six (6) each #5035 Platelet Wash Solution one Liter, one (1) each to be delivered every other month. Please provide pricing for the items listed above as (1) price per package x number of shipment. Please provide these prices separately for the base year Lot II Option I and Lot II Option III. Pursuant to FAR 17.203(d) bidders may offer varying prices for option periods. All shipments requested to be sent FOB Destination to the National Naval Medical Center, Bethesda, MD 20889. The provision at FAR 52.212-1 ``Instructions to Offerors-Commercial Items'' is hereby included by reference. The provision at FAR 52.212-2 ``Evaluation-Commercial Items'' applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: price, technical capability, delivery, options and past performance. Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, point of contract with telephone numbers and other relevant information. The following provisions are incorporated by reference, FAR 52.212-4 ``Contract Terms and Conditions-Commercial Items'' and FAR 52.217-5 ``Evaluation of Options''. The clause at 52.217-9 ``Option to Extend the Term of the Contract'' is incorporated by full text. (a) The Government may extend the term of this contract by written notice to the Contractor within 10 days; provided, that the Government shall give the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option provision. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed three (3) years.
