This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are are being requested
and a written solicitation will not be issued. Solicitation number
578-41-96 is issued as a Request For Proposal (RFP). This solicitation
document, incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 90-39. Hines Veterans Affairs
Hospital (VAH) intends to purchase one each, Simulix-MC Upgrade, or
equal, to upgrade Hines VAH owned Simulix-Y Radiation Therapy Simulator
to a Simulix-MC Radiation Therapy Simulator, from the Nucletron-Oldelft
Corp. Purchase to also include one each, Eigen Digital-8 Digital Image
Recorder/Processor. Upgrade will transform current simulator into a 32
bit microprocessor controlled simulator with new remote console and
table side control panels that utilize easy to use toggle switches,
provide two (2) 14'' digital video display monitors for all parameters,
including the ability to enter patient demographics, and laser printer
for hard copy printout. Upgrade will provide as additional features;
standard set ups, opposed field function, treatment machine profiling,
active RS232 port, state of art logic anti-collision,
calibration/diagnostic service kit, new collimator with motorized
movement, including symmetrical and asymmetrical movement of
delineating wires and collimator blades, shutter tracking for both the
delineator wires and image intensifier and newest 100 cm reticule
optical and mechanical focus-to-skin indication. Equivalent equipment
upgrade must be compatible and correlatable to to the existing Hines
VAH owned Simulix-Y Simulator and provide all necessary hardware and
software to upgrade to a Simulix-MC simulator. Delivery is to be F.O.B.
destination within 60 days of contract award to Hines VAH Supply
Warehouse, Bldg. 220, Hines, IL 60141. The provisions at 52.212-1,
Instructions to Offerors-- Commercial Items, applies to this
acquisition with no addenda. In accordance with the provisions at
52.212-2, Evaluation--Commercial Items, the Government will award a
contract resulting from this solicitation to the responsible offeror
whose offer, conforming to the solicitation, will be the most
advantageous to the Government, price and other factors considered. The
following factors, in a descending order of importance, will be used to
evaluate offers of equivalent equipment upgrade: (1) Compatibility and
correlatability to Hines VAH Simulix-Y Simulator with the ability to
upgrade to a Simulix-MC simulator (2) Conformation to specifications
(3) Price (4) Warranty on upgraded portion of simulator. Offerors are
to include a completed copy of the provisions at 52.212-3, Offerors
Representations and Certifications-- Commercial Items, with offer. The
clause at 52.212-4, Contract Terms and Conditions--Commercial Items,
applies to this acquisition with no addenda. The clause at 52.212-5,
Contract Terms and Conditions Required to Implement Statues or
Executive Orders--Commercial Items, applies to this acquisition with
none of the additional FAR clauses cited in the clause applicable to
this acquisition. Offers of equivalent equipment upgrade are to be
submitted on August 23, 1996 by Close of Business (4:00 PM Central
Standard Time) to Contracting Officer (90C4), Edward Hines Jr. Veterans
