Q -- DELIVERY OF HOME OXYGEN AND DURABLE MEDICAL EQUIPMENT SOL 626-16-97 DUE 091696 POC Susan Hilley, 615-327-5312 Furnish Home Oxygen Therapy on a monthly basis for VA beneficiaries of the Department of Veterans Affairs Medical Center, Nashville and Murfreesboro, Tennessee. Contractor shall provide oxygen concentrators and accessory equipment as needed. personnel providing service under this contract will be Registered Respiratory Therapist or Certified Respiratory Therapy Technician licensed in the State of Tennessee and/or Kentucky when delivering in that state. In addition, furnish storage, parts and labor to repair, clean, deliver and pickup of Government-owned durable medical equpment for the VA Medical Centers, Nashville and Murfreesboro, Tennessee. Contractor shall have a storage facility inside Davidson County, Nashville, TN or the surrounding counties to store VA equipment which will be physically inventoried on a weekly basis by the VA and the Contractor. Contractor must have a sufficient stock of frequently used parts for repairs. Service representatives shall have documented training to perform all services required. VA beneficiaries to be served by this contract are located throughout Middle Tennessee and surrounding areas including nearby counties in Kentucky. Contractor is required to be accredited by the Joint Commission on Accreditation of Health Care Organizations (JCAHO) on these types of businesses.
