65 -- PREFERRED PROVIDER AGREEMENT FOR MEDICAL & SURGICAL SUPPLIES SOL 527-47-96 DUE 090996 POC Mr. Perry Danner, Acquisition & Material Manager, Telephone No. 718 630-3550 This serves as notification of the Government's intent to negotiate preferred provider agreements for medical and surgical supplies with Baxter, Kendall and Medline Corporations for the following Veterans Administration facilities in VISN 3: VAMC Bronxs, NY; VAMC Brooklyn, NY; VAMC Castle Point, NY; VAMC East Orange, NJ; VAMC Lyons, NJ; VAMC Montrose, NY and VAMC New York, NY - associated satellite clinic second divisions are also included. The delivery requirements shall be 24-48 hours after receipt of each order. A one year agreement with three (3) renewal options is anticipated.
