Howard,MD, Martinsburg, WV, Perry Point, MD and Washington, DC, intend to award multiple requirements contracts to firms seeking to become preferred providers of artificial limbs and related services to eligible beneficiaries within the following catchment areas: Pennsylvania (3 Counties - Fulton, Franklin, Adams), West Virginia (8 Counties - Mineral, Hampshire, Morgan,Berkeley, Jefferson, Grant, Hardy, Pendleton), Virginia (10 Counties - Shenandoah, Warren, Page Rappahannock, Fauquier, Clarke, Loudoun, Fairfax, Prince William, Arlington), and the entire state of Maryland (EXCLUDING the following counties Dorchester, Caroline, Wycomico, Worchester, Somerset). Each contract will be for one (1) base year and provide for two (2) renewable option years, subject to the availability of funds. The contractor's facilities shall be approved by the Facility Accreditation Program of the American Board for Certification in Orthotics and Prosthetics, Inc., or have successfully passed inspection by VA prior to award of a contract. All contractors shall have full-time American Board Certified or VA Qualified prosthetist on staff at each of their locations. All requests for this solicitation must be made in writing.
