J -- SERVICE/MAINTENANCE CONTRACT FOR MHTI CT SIMULATOR SOL RFP 527-5-97 DUE 092396 POC Jo Ann Petrillo, Contract Specialist, FAX 718 630-3558, PHONE 718 630-3566 The Brooklyn Veterans Administration Medical Center is seeking to negotiate a (1) year term contract to run from 1 October 1996 thru 30 September 1997 to provide full service and maintenance for a MHTI CT SIMULATOR. Services to be provided include furnishing all labor, tools, equipment, transportation and parts (excluding glassware) necessary to perform diagnosis, calibration, repairs and preventive maintenance on the unit. All services shall be performed by manufacturer trained technicians. The contract will include assessment of liquidated damages for loss of production due to unit downtime.
