This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Simplified Acquisition Procurement (SAP). The solicitation number is N00168-97-Q-L014 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-37. This procurement is unrestricted. The Standard Industrial Code (SIC) for this acquisition is 3069. The industry size standard is 500 employees. The Laboratory Medicine Department, National Naval Medical Center, Bethesda, MD requirement is for ``Manufacturer/Safeskin brand name only''. NNMC intends to award a sole source (product only) standing order to an offeror in compliance with the requirements of this solicitation. Standing Order will be for the period 1 October 1996 through 30 September 1997. Description of items to be procured as follows: (1) Three hundred twenty-four (324) cases Catalog# SSC220 Small latex powder-free hypoallergenic gloves, 100/bx, 10 bx/case, 1000/cs, Twenty-seven cases to be delivered every month; (2) Five hundred seventy-six (576) cases Catalog# SSC330 Medium latex power-free hypoallergenic gloves, 100/bx, 10 bx/case, 1000/cs, Forty-eight (48) cases to be delivered every month; (3) Three hundred ninety-six (396) cases Catalog# SSC440 Large latex powder-free hypoallergenic gloves, 100/bx, 10 bx/case, 1000/case, thirty-three (33) cases to be delivered every month. Please provide pricing for the items listed above as (1) unit price per case x total quantity. All shipments requested to be shipped FOB Destination to the National Naval Medical Center, Bethesda, MD 20889. The provision at FAR 52.212-1 ``Instructions to Offerors-Commercial Items'' is hereby included by reference. The provision at FAR 52.212-2 ``Evaluation-Commercial Items'' applies to this acquisition. The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: price, delivery and past performance.
