This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in Subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotes are being requested and a
written solicitation will not be issued. This is a Simplified
Acquisition Procurement (SAP). The solicitation number is
N00168-97-Q-L069 and is issued as a request for quotation (RFQ). This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 90-37. This procurement
is unrestricted. The Standard Industrial Codes (SIC) for this
acquisition are 2819, 3089 and 3231. The industry size standards are
1000 employees for 2819, 3089 and 3231. The industry size standards are
1000 employees for 2819 and 500 employees for 3089 and 3231. Offerors
shall provide a description, manufacturer's brand and part number of
the product in detail to ensure compliance with the requirements of
solicitation. Standing Order for a base period of one (1) year (Lot 1
Base Year) effective 1 October 1996 through 30 September 1997 with four
(4) year year options. Lot II - Option 1, period of performance 1
October 1997 through 30 September 1998. Lot III - Option II, period of
performance 1 October 1998 through 30 September 1999. Lot IV - Option
III, period of performance 1 October 1999 through 30 September 2000.
Lot V - Option IV, period of performance 1 October 2000 through 30
September 2001. Description of items to be procured as follows:
Manufacturer Surgipath Medical Industries (1) Twenty-four (24) cases
Catalog# 3100 Frostbite (for rapid freezing of fresh or parafin
embedded tissue), six (6) 12 ounce cans per case, two (2) cases to ben
delivered every month; (2) One hundred twenty (120) cases Catalog #
00145 24x50 Grade 1 coverglass coverslip 10 oz/case, ten (10) cases to
be delivered every month; (3) Twenty-four (24) each Catalog# 1520 Gill
(II) Hematoxylin, one gallon container, two 92) gallons to be delivered
every month; (4) Twelve (12) each Catalog# 1600 Eosin, one gallon
container, one (1) gallon to be delivered every month; (5) Thirty-six
(36) cases Catalog# 02301 White processing cassettes without lids (hold
tissue specimen), 1000/case, three (3) cases to be delivered every
month. Please provide pricing for the items listed above as (1) price
per unit x total quantity. Please provide these prices separately for
the Base Year, Lot II Option 1, Lot III Option II, Lot IV Option III
and Lot V Option IV. Pursuant to FAR 17.203(d) bidders may offer
varying prices for option periods. All shipments requested to be sent
FOB Destination to the National Naval Medical Center, Bethesda, MD
20889. The provision at FAR 52.212-1 ``Instructions to
Offerors-Commercial Items'' is hereby included by reference. The
provision at FAR 52.212-2 ``Evaluation-Commercial Items'' applies to
this acquisition. The Government will award a contract resulting from
this solicitation to the responsible offeror whose offer conforming to
the solicitation will be most advantageous to the Government, price an
other factors considered. The following factors shall be used to
evaluate offers: price, technical capability, delivery, options and
past performance. Past performance information, when included as an
evaluation factor, to include recent and relevant contracts for the
same or similar items and other references (including contract numbers,
points of contact with telephone numbers and other relevant
information. The following provisions are incorporated by reference,
FAR 52.212-4 ``Contract Terms and Conditions-Commercial Items'' and FAR
52.217-5 ``Evaluation of Options''. The clause at 52.217-9 ``Option to
Extend the term of the Contract'' is incorporated by full text. (a)
The Government may extend the term of this contract by written notice
to the contractor within 10 days; provided, that the Government shall
give the Contractor a preliminary written notice of its intent to
extend at least 60 days before the contract expires. The preliminary
notice does not commit the Government to an extension. (b) If the
Government exercises this option, the extended contract shall be
considered to include this option provision. (c) The total duration of
this contract, including the exercise of any options under this
clause, shall not exceed (5) years. The clause 52.223-3 ``Hazardous
Material Identification and Material Safety Data'' is incorporated by
reference. Offers are due by COB 8/27/96 at Acquisition Management
