This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in FAR Subpart 12.6, as supplemented
with additional information included in this notice. This announcement
constitutes the only solicitation; bids are being requested and a
written solicitation will not be issued. Solicitation number
DADA08-96-B-0014 applies and is issued as an Invitation For Bids
(IFBs). This combined synopsis/solicitation and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-38 and Defense Acquisition Circular 91-10. The standard industrial
classification code is 8011 and the business size is $3.5 Million.
Contractor shall provide Urology Physician Services on site at Winn
Army Community Hospital (WACH), Fort Stewart, Georgia. Contractor shall
perform medical and surgical services of nature and scope in accordance
with the precepts of the American Academy of Urology, the standards of
the Joint Commission for Accreditation of Healthcare Organizations
(JCAHO), and the professional standards of Winn Army Community Hospital
(WACH), Fort Stewart, Georgia. This requirement is for personal
services and the performance of the contract physician is subject to
the day-to-day supervision and control of the Chief, Surgery
Department. This supervision is comparable to that exercised over
military and civil service healthcare providers engaged in comparable
work. Any personal injury claims alleging negligence by the contract
physician within the scope of the healthcare providers performance of
this contract shall be processed by DoD the same as claims alleged
against government personnel. PERFORMANCE PERIOD: This requirement is
for a firm fixed price contract that includes a base year and four (4)
one (1) year option periods with the option to extend services for up
to six months after contract expiration. Offerors must submit pricing
for the base year and each option period. Offers for the base year and
each option periods can vary in price. ITEM 0001, Urology Physician
Services, Base Year (1 Oct 96-30 Sep 97), Quantity 2000 Hours; ITEM
0002, Urology Physician Services, First Option Period (1 Oct 97-30 Sep
98), Quantity 2000 Hours; ITEM 0003, Urology Physician Services,
Second Option Period (1 Oct 98-30 Sep 99), Quantity 2000 Hours; ITEM
0004, Urology Physician Services, Third Option Period (1 Oct 99-30 Sep
00), Quantity 2000 Hours; ITEM 0005, Urology Physician Services,
Fourth Option Period (1 Oct 00-30 Sep 01), Quantity 2000 Hours.
QUALIFICATIONS. The urologist shall meet the following minimum
qualifications: 1. Hold a Doctor of Medicine Degree (M.D.) from an
accredited college approved by the Council on Medical Education and
Hospitals of the American Medical Association, or Doctor of Osteopathy
Degree (D.O.) from a college accredited by the American Osteopathic
Association; 2. Contract Physicians shall have as a minimum, one of the
past three years in active urology practice; 3. Residency/training is
considered equivalent to active urology practice; 4. Have completed an
urology residency training program which is accredited by the American
Board of Urology; 5. Foreign medical graduate physicians shall possess
a valid certificate from the Educational Council for Foreign Medical
Graduates; 6. Shall have and maintain current Basic Cardiac Life
Support (BCLS) or Advanced Cardiac Life Support (ACLS) certification;
7. OSHA Occupational Exposure to Bloodborne Pathogens Training; 8. DEA
Controlled Substance Registration Certificate. CREDENTIALS: 1. All
physicians providing services under the terms and conditions of this
contract shall have their credentials reviewed by the Credentials
Committee of the MTF and be granted hospital privileges by the MTF
commander in accordance with AR 40-68 and local MEDDAC regulations
prior to commencement of services. 2. Applications for clinical
privileges shall be submitted by the contractor with their offer. Fax
copies may be accepted with originals mailed. All documents shall have
primary source verification by the contractor. 3. Should the
contractor need to add additional physicians during the contract
period, the same credentialing process shall apply for each additional
physician. 4. The contractor must comply with requests for informal
interviews with the contractor's physician(s). The credentials
committee may request an informal interview over the telephone or
present questions in writing to the contractor's physician. 5.
Credentials packets are available upon request. HEALTH REQUIREMENTS:
All contract physicians, including backup physicians, performing
services under this contract shall comply with the all MTF health and
immunization requirements. The expense for all physical examinations to
comply with the health requirements shall be borne by the contractor at
no additional cost to the government. HOURS OF PERFORMANCE: Regular
Duty Hours shall be Monday through Friday, 7:30 a.m.to 4:30 p.m.,
excluding legal Federal holidays or a schedule that is agree able to
both the government and the contractor. Duty hours include a one hour
lunch period, which is not paid by the government. The contractor
physician shall perform outpatient services in the Urology Clinic and
surgery as scheduled by the Chief, Surgery Service. The physician shall
conduct daily rounds on all inpatients for which he/she is the primary
physician. ON-CALL. No routine call will be required for this
contract. SPECIFIC TASKS: Contractor shall perform medical examination
and treatment in the surgical specialty of urology. The contractor
shall provide medical care to all beneficiaries authorized care in AR
40-3 and shall perform all required emergency urology surgical health
care procedures during normal clinic. The contractor shall perform
elective surgery only as delineated in clinical privileges granted by
the Commander, Winn Army Community Hospital. Workload: Outpatient
Workload: The contractor shall see primary care patients scheduled by
the Chief, Surgery Service. The Government will schedule primary care
urology patients at a rate of up to four appointments per hour.
Patients will range from pediatric to geriatric with a full range of
urology maladies. Inpatient Workload: The contractor shall perform
surgery as scheduled by the Chief, Surgery Service. The contractor
shall perform surgical procedures to include, but not limited to:
Exploration of Kidney, Removal of Kidney, Partial Removal of Kidney,
Re-implant ureter in Bladder, Partial removal of Bladder, Irrigation of
Bladder, Simple Cystometrogram, Attach Bladder/Urethra, Repair of
Bladder neck, Cystoscopy and Treatment, Cystoscopy and Ureter
Catherization, Prostatectomy (TURP), Incision of Urethra, Dilation of
Urethra stricture, Dilation of Urethra, Circumcision, Biopsy of Testis,
Suspension of Testis, Remove Epididymis lesion, Drainage of Hydrocele,
Removal of Hydrocele, Repair of Hydrocele, Removal of sperm duct(s),
Biopsy of Prostate, Extensive Prostate surgery. FAR Provision 52.212-1,
