DADA08) This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; bids are being
requested and a written solicitation will not be issued. Solicitation
number DADA08-96-B-0013 applies and is issued as an Invitation For Bids
(IFBs). This combined synopsis/solicitation and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-38 and Defense Acquisition Circular 91-10. The standard industrial
classification code is 8011 and the business size is $3.5 Million.
Contractor shall provide Dermatology Physician Services on site at
Lyster Army Community Hospital (LACH, Fort Rucker, Alabama. Contractor
shall perform Dermatology Physician Services of nature and scope in
accordance with the precepts of the American Academy of Dermatology,
the standards of the Joint Commission for Accreditation of Healthcare
Organizations (JCAHO), and the professional standards of Lyster Army
Community Hospital (LACH), Fort Rucker, Alabama. This requirement is
for personal services and the performance of the contract physician is
subject to the day-to-day supervision and control of the Chief,
Dermatology Clinic. This supervision is comparable to that exercised
over military and civil service healthcare providers engaged in
comparable work. Any personal injury claims alleging negligence by the
contract physician within the scope of the healthcare providers
performance of this contract shall be processed by DoD the same as
claims alleged against government personnel. PERFORMANCE PERIOD: This
requirement is for a firm fixed price contract that includes a base
year and four (4) one (1) year option periods with the option to extend
services for up to six months after contract expiration. Offerors must
submit pricing for the base year and each option period. Offers for
the base year and each option periods can vary in price. ITEM 0001,
Dermatology Physician Services, Base Year (1 Oct 96-30 Sep 97),
Quantity 1256 Hours; ITEM 0002, Dermatology Physician Services, First
Option Period (1 Oct 97-30 Sep 98), Quantity 1256 Hours; ITEM 0003,
Dermatology Physician Services, Second Option Period (1 Oct 98-30 Sep
99), Quantity 1256 Hours; ITEM 0004, Dermatology Physician Services,
Third Option Period (1 Oct 99-30 Sep 00), Quantity 1256 Hours; ITEM
0005, Dermatology Physician Services, Fourth Option Period (1 Oct 00-30
Sep 01), Quantity 1256 Hours. QUALIFICATIONS. The dermatologist shall
meet the following minimum qualifications: 1. Hold a Doctor of Medicine
Degree (M.D.) from an accredited college approved by the Council on
Medical Education and Hospitals of the American Medical Association, or
Doctor of Osteopathy Degree (D.O.) from a college accredited by the
American Osteopathic Association; 2. Contract Physicians shall have as
a minimum, Board Eligible in Dermatology and licensed to practice
medicine in Alabama; 3. shall have completed a residency training
program which is accredited by the American Board of Dermatology; 4.
Foreign medical graduate physicians shall possess a valid certificate
from the Educational Council for Foreign Medical Graduates; 5. Shall
have a current Basic Cardiac Life Support (BCLS). 6. Shall have
received training in occupational exposure to bloodborne pathogens. 7.
Shall have a controlled substances registration certficate from the
Drug Enforcement Administration (DEA). CREDENTIALS: 1. All physicians
providing services under the terms and conditions of this contract
shall have their credentials reviewed by the Credentials Committee of
the MTF and be granted hospital privileges by the MTF commander in
accordance with AR 40-68 and local MEDDAC regulations prior to
commencement of services. 2. Applications for clinical privileges shall
be submitted by the contractor with their offer. Fax copies may be
accepted with originals mailed. All documents shall have primary source
verification by the contractor. 3. Should the contractor need to add
additional physicians during the contract period, the same
credentialing process shall apply for each additional physician. 4. The
contractor must comply with requests for informal interviews with the
contractor's physician(s). The credentials committee may request an
informal interview over the telephone or present questions in writing
to the contractor's physician. 5. Credentials packets are available
upon request. HEALTH REQUIREMENTS: All contract physicians, including
backup physicians, performing services under this contract shall comply
with the all MTF health and immunization requirements. The expense for
all physical examinations to comply with the health requirements shall
be borne by the contractor at no additional cost to the government.
HOURS OF PERFORMANCE: Regular Duty Hours shall be 21 hours per week
(seven hours on Tuesday, Thursday and Friday, exclusive of federal
holidays or as agreed upon by the contractor and Contracting Officer's
Representative (COR). Duty shall be performed between 7:30 a.m.to 4:30
p.m. Duty hours include a one hour lunch period, which is not paid by
the government. The contractor physician shall perform outpatient
services in the Dermatology Clinic as scheduled by the Chief,
Dermatology Clinic. ON-CALL. No routine call will be required for this
contract. SPECIFIC TASKS: Contractor shall perform medical examination
and treatment in the specialty of Dermatology. The contractor shall
perform surgical procedures to include, but not limited to: Acne,
Dermatologic Abscesses, Dermatologic Drug Reactions, Dermatologic
Manifestions of Septicemic Disease, Fungal Infections, Impetigo, Viral
and Bacterial Exanthena and warts. The physician performing services
under this contract shall have sufficient experience to be able to
adequately perform medical procedures to include, but not limited to
the following: Acne/Accutane Therapy, Cauterization, Comedone
Extraction, Cryotherapy, Cultures, Curettage with/without, Excisional
Biopsy, I&D Abscess, Podophyllum Treatment, Punch Biopsy, Scar
Revision, Shave Biopsy, Suture Removal, Suturing, Trichloroactic Acid
Treatment, Use 1% and 2% Lidocane, Epinephrine, Venipuncture, Wound
Debridgement and Wound Dressings. FAR Provision 52.212-1, Instructions

Evaluation of Commercial Items, the government intends to make a single
award to the responsible offeror with the lowest price. NOTE: All
offerors are required to submit a completed copy of FAR Provision
