Q -- MEDICAL SERVICES SOL DACW39-96-R-0042 DUE 100296 POC Contract Specialist SHIRLEY JO MILLER (601) 634-3557 (Site Code DACW39) On site physician services necessary to provide occupational safety and health support for civilian and military employees at Waterways Experiment Station, Lower Mississippi Valley Division Corps of Engineers, and the Vicksburg District Corps of Engineers. Services performed shall be in accordance with Army Regulation 40-3, Medical Dental, and Veterinary Care, and Army Regulation 40-5, Preventive Medicine. Services will be for four (4) hours each day Monday through Friday, holidays excluded. Services shall be preformed at Waterways Experiment Station between the hours of 8:00 A. M. and 12:00 NOON on Mondays, Wednesdays and Fridays and the Vicksburg District between the hours of 7:30 A. M. and 11:30 A. M. on Tuesdays, Wednesdays and Thursdays. Physicians performing these services shall be licensed medical doctors who are qualified to practice in the Vicksburg, MS area, and shall have education, training, or experience in preventive medicine or in occupational health.
