Musgrove, 301/295-6754. This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued. This is
a simplified acquisition procurement (SAP). The solicitation number is
N00168-97-Q-L082 and is issued as a request for quotation (RFQ). This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 90-37. The procurement
is unrestricted. The Standard Industrial Code (SIC) for this
acquisition is 3089. Offerors shall provide a description,
manufacturer's brand and part number of the product in detail to ensure
compliance with the requirements of the solicitation. Standing order
for a base year period of one (1) year (Lot I Base Year) effective 1
October 1996 through 30 September 1997 with one (1) year option. Lot II
- Option I, period of performance 1 October 1997 through 30 September
1998. Description of items to be procured as follows: (1) One hundred
forty-four (144) cases Catalog# 367986 Plus SST brand tubes with
hemogard closure, clot activator and gel for serum separation, silicone
coated, interior gold closure, paper label, 13 x 75 vol. draw 3.5ml
venous blood collection system, sterile vacutainer bran plus tubes
(plastic), 1000/case, twelve (12) cases to be delivered every month;
(2) Sixty (60) cases Catalog# 367861 plus tubes with hemogard closure
for whole blood determination, K2 EDTA (spray dried), silicone coated
and interior, lavender closure, 13 x 75, 7.2mg K2 EDTA, vol. draw tubes
(plastic), 1000/case, five (5) cases to be delivered every month; (3)
Twelve (12) cases Catalog# 367878 plus tubes with hemogard closure
requiring plasma-sodium heparin (spray dried), green closure, paper
label, 13 x 100, 145 U.S.P. units sodium heparin, vol. draw 6ml, venous
blood collection systems, sterile vacutainer brand plus tubes
(plastic), 1000/case, one (1) case to be delivered every month; (4)
Twelve (12) cases Catalog# 367722 sterile tubes, non-siliconized
glycolytic inhibiter, lt. grey closure, paper label, 13 x 75, 6mg pot.
oxalate, 7.75mg sod. fluoride, vol. draw 3ml, venous blood collection
systems sterile vacutainer brand evacuated tubes with hemogard
closure, 1000/case, one (1) case to be delivered every month; (5)
Twelve (12) cases Catalog# 369704 sterile tubes for coagulation
studies, .129M buffered sod. citrate (equiv. to 3.8% sod. cit.),
siliconized interior, blue closure, paper label, 13 x 75, 0.5ml
solution (16.0mg sod. cit., 2.1mg cit. acid), vol. draw 4.5ml, venous
blood collection systems, sterile vacutainer brand evacuated tubes with
hemogard closure, 1000/case, one (1) case to be delivered every month.
Please provide pricing for the items listed above as (1) price per
case x total quantity. Please provide these prices separately for the
Base year and Lot II Option I. Pursuant to FAR 17.203(d) bidders may
offer varying prices for option periods. All shipments requested to be
sent FOB Destination to the National Naval Medical Center, Bethesda,
MD 20889. The provision at FAR 52.212-1 ``Instructions to
Offerors-Commerical Items'' is hereby included by reference. The
provision at FAR 52.212-2 ``Evaluation-Commercial Items'' applies to
this acquisition. The Government will award a contract resulting from
this solicitation to the responsible offeror whose offer conforming to
the solicitation will be most advantageous to the Government, price
and other factors considered. The following factors shall be used to
evaluate offers: price, technical capability, delivery, options and
past performance. Past performance information, when included as an
evaluation factor, to include recent and relevant contracts number,
points of contact with telephone numbers and other relevant
information. The following provisions are incorporated by reference,
FAR 52.212-4 ``Contract Terms and Conditions-Commercial Items'' and FAR
52.217-5 ``Evaluation of Options''. The clause at 52.217-9 ``Option to
Extend the Term of the Contract'' is incorporated by full text. (a)
The Government may extend the term of this contract by written notice
to the contractor within 10 days; provided, that the Government shall
give the Contractor a preliminary written notice of its intent to
extend at least 60 days before the contract expires. The preliminary
notice does not commit the Government to an extension. (b) If the
Government exercises this option, the extended contract shall be
considered to include this option provision. (c) The total duration of
this contract, including the exercise of any options under this
clause, shall not exceed two (2) years. Offers are due 8/30/96 by COB
