This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a simplified acquisition procurement. The solicitation number is N00168-97Q-2L15 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-37. This procurement is unrestricted. The Standard Industrial Code (SIC) for this acquisition is 3841. Offerors shall provide a description, manufacturer's brand, model number and part number of the product in detail to ensure compliance with the requirements of the solicitation. Description of items to be acquired: CLIN 0001: (240 ea) 4 Port Manifold kit w/contrast saver, large bore Y adaptor, torque device, and wire introducer and with three (3) color syringes - 1 white labeled ``HEPARIN'', 1 yellow labeled ``NITRO'', and 1 blue labeled ``VERAPMIL'' (Delivery of 20 ea the first week of every month); CLIN 0002: (840 EA) 3 Port manifold kit w/contrast saver (Delivery of 70 ea the first week of every month). All shipments will be sent FOB destination to the Cardiac Cath Lab, National Naval Medical Center, Bethesda MD 20889.
