Responses must contain sufficient documentation to establish a bonafide capability to fulfill the requirement. The quotation number is N62645-96-Q-1011 and is issued as a Request for Quotation. Liebel-Flarsheim Company, 2111 East Galbraith Road, Cincinnati, Ohio 45215 shall satisfy this requirement by providing the following contract line items (CLINS): CLIN 0001, Pre-installation visit, Qty 1 lot; CLIN 0002, Installation, Qty 1 lot; CLIN 0003, Urology Technicians Training, Qty 1 lot; CLIN 0004, Biomedical Repair Technicians Training, Qty 1 lot.
