Verhulst, (216) 838-6068 The VA Medical Center, Brecksville, OH, intends to contract for Scarce Medical Specialist, Part-Time Physical Therapy Services to and at the VA Medical Center Outpatient Clinic, Youngstown, OH. Contractor shall be required to carry out patient treatment in accordance with established guidelines of the state of Ohio. Duties include initial evaluation of all physician referred patients for physical therapy, development of an individual Health Plan (IHP) program, maintenance of patient records, evaluation and therapy of patients referred to the PT staff for management of chronic musculoskeletal conditions, post traumatic rehabilitation, functional rehabilitation, and any other therapy associated with physical therapy. Contract Physical Therapist shall be responsible for supervision of other PT staff members provided by the contractor.
