synopsis/solicitation for commercial items prepared in accordance with
the format in subpart 12.6, as supplemented with additional
information included in this notice. This constitutes the only
solicitation. Proposals are being requested and a ''Written
Solicitation'' will not be issued. Solicitation RFP 506-19-97 is issued
as a request for proposal and incorporates clauses that are in effect
through Federal Acquisition Circular 37. Contractor shall furnish and
install F.R.E.D. (Friendly Roll-About Engineered for Doctors)
Telemedicine System to the Department of Veteran Affairs, VA Medical
Center, 2215 Fuller Road, Ann Arbor, MI., 48105, and the Department of
Veteran Affairs, Toledo Outpatient Clinic, 3333 Glendale Avenue,
Toledo, OH 43614, FOB destination within 45 days of award date. System
to include, but not be limited to one (1) ea FRED-B Telemedicine
Video/Audio Cart; one ea (1) FRED-ARM High Resolution Camera on
articulating arm, Hitachi 3 chip CCD; one (1) AMD/LCI200 Welch-Allyn
light, video camera; one (1) ea AMD 2030 Welch-Allyn 47370 video
endoscope, one (1) ea AMD 2020 Welch-Allyn 14680; one (1) ea AMD 2020B
Welch-Allyn 72200 replacement battery; one (1) ea AMD 3200 Stethoscope
(for TOPC); one (1) ea VTEL 127TCd Telemedicine/conferencing cart
system; one ea (1) AMD DP524 Stethoscope (for Ann Arbor VA); one (1) ea
ELMO document stand, cable; two (2) ea Aux-Vtel interface cables;
installation of telemedicine FRED at TOPC; installation of telemedicine
127TCd at VAMC Ann Arbor, MI., three (3) year warranty extension on
FRED at TOPC; three (3) year warranty extension on 127TCd at Ann Arbor,
MI., VAMC; 60 - 90 minutes of training at Ann Arbor, MI., VAMC and TOPC
at time of installation, and two (2) ea service manuals. Brand name or
equal items may be considered if descriptive literature is provided
with proposal. The provision at 52.212-1, Instruction to
Offerors--Commercial Items and 52.212-2, Evaluation-Commercial Items
applies to this acquisition. The following factors shall be used to
evaluate offers: (1) Technical (capability of the offered system to
meet agency needs); (2) Price; (3) Past performance; (4) Warranty; (5)
On-site training; (6) Delivery time. A completed copy of the provision
at 52.212-3, Offerors Representations and Certifications--Commercial
Items should be submitted with the proposal. Clauses 52.212-4 Contract
Terms and Conditions-Commercial Items, and 52.232-28 Electronic Funds
Transfer Payment Methods, applies to this solicitation. Warranty
extensions should include parts and labor. Clause 52.212-5, Contract
Terms and Conditions Required to Implement Statutes or Executive
Orders--Commercial Items, applies to this acquisition, including
52.222-3, 52.233-3, 52.203-6, 52.203-10, 52.219-8, 52.219-9, 52.222-26,
52.222-35, 52.222-36, 52.222-37, and 52.225-21. Please submit proposals
to Contracting Officer (90C), Department of Veteran Affairs, VA Medical
Center, 2215 Fuller Road, Ann Arbor, MI., 48105, citing solicitation
506-19-97 by close of business, September 26, 1996. All responsible
sources may submit a proposal which will be considered by the agency.
