synopsis/solicitation for commercial items prepared I/a/w the format
prescribed in sub-part 12.6 of the FAR, as supplemented with additional
information included in this notice. This announcement constitutes the
solicitation for the required services. Proposals are hereby requested
under N00140-96-R-N187, a subsequent solicitation document will not be
issued. FISC Det Phila intends to acquire on a full and open
competitive basis for mobile MRI services for up to 15 MRI scans a day,
two days a week in support of the Naval Hospital Great Lakes, (NHGL)
IL. Award of a firm fixed price contract is the expected result of this
action. The Government's obligation is contingent upon Congress
enacting FY97 appropriations. The SIC is 8049 Requirements: 0001 Base
period Mobile MRI scans at the NHGL 2 days a week, 104 days a year,
starting 1 Oct 96 or date of award which ever is later through 30 Sept
97. 0002 Option year (1), Mobile MRI scans at the NHGL 2 days a week,
104 days a year, starting 1 Oct 97 through 30 Sept 98. 0003 Option
year (2), Mobile MRI scans at the NHGL 2 days a week, 104 days a year,
starting 1 Oct 98 through 30 Sept 99. 0004 Opton year (3), Mobile MRI
scans at the NHGL, 2 days a week, 104 days a year, starting 1 Oct 99
through 30 Sept 00. The Government has the unilateral right to exercise
the option periods listed above provide that the Government notifies
the contractor of the Government' s intent to exercise the option
within 30 days prior to the expiration of the current period of
performance. The MRI services performed in response to the requirement
will be performed to the higheststandards in the MRI profession. All
services shall be rendered by individuals fully qualified in the MRI
profession, and holding any licenses required by law. The required
services are as follows: The contractor will not use as a consultant or
employ (on either a full or part time basis) any current Department of
Defense (DoD) personnel (civilian or military) without the prior
approval of the Contracting Officer. The Contractor shall provide
professional services and portable facility to operate a Magnetic
Resonance Imager (MRI) for performance of body and extremity exams on
beneficiaries at the Naval Hospital, Great lakes (NHGL), Illinois.
Services are required to support the Radiology Department Imaging
Branch. Services include the Imager with its mobile unit. NHGL will
furnish film, film processing, contract agents, liners, utilities,
phone lines and staging site. The functions performed by the contractor
under the terms of the contract will include, but not be limited to the
following: 1. A 1.0 telsa strength magnet with shielding as long as the
magnet can be sited at NHGL, and all standard surface coils to
accommodate body, head, neck, and extremity studies. 2. Support unit
that provides patient waiting room and patient changing room. 3. A
laser camera for transferring images to film. Images will become the
property of NHGL. 4. Performance of exam which shall consist of a
single body area and the number and type of pulsing sequence to be
defined by the radiologist. Injection of contrast media and consequent
pulsing sequences are included as part of the examination with extra
charges. NHGL's radiologist will determine who will inject contrast
media at the time of the examination, the contractors MRI operator will
need to be qualified to inject contrast media. 5. Conduct or provide
training sessions in the interpretation of MRI exams for 4
radiologist's. 6. Process films and provide them to the radiologist on
duty immediately following each examination. Contractor's technologist
will release the patient once approval of the film has been given by
the staff radiologist. 7. If motion by the patient occurs during the
initial sequence, the radiologist is to be immediately notified. It
will be at the radiologist's discretion whether or not to continue and
the completed scan results are poor due to continued patient motion,
compensation will be made to the contractor. NHGL will not be
responsible for, nor pay for repeats due to contractor error. The
quality of the final product will be determined by the NHGL's
Radiologist. 8. Equipment: The contractor shall propose a 1.0 telsa
strength magnet and also certify that the proposed Mobile MRI system
will not be more than five years old at anytime during the contracts
period of performance. (This certification must be in writing) 8.1 The
contractor will furnish and be equipped with both nonferrous and
stretcher for patient transportation which will be used as required as
determined by patient's condition. Unit should be able to support
patients who weight up to 350 lbs. The stretcher and wheelchair are to
be be adult size and in good condition. The maintenance, repair, and
labeling of wheelchair and stretcher are the responsibility of the
contractor. 9. Patient Transport: Transporting patients to be and from
the MRI staging site will be the responsibility of the contractor. 10.
Personnel: The contractor will supply one (1) fully qualified
technician to operate the unit, and one (1) fully qualified Mobile MRI
Truck operator to transport the mobile MRI unit to and from NHGL. The
truck driver will also assist in Patient transport. The successful
contractor will have to provide the following certifications (if
applicable) prior to contract performance: 1. Proof of Certification as
an A.R.T from an Institution recognized from the American Medical
Association. 2. Proof of their certification in Cardiopulmonary
Resuscitation CPR basic life support. 3. Proof of Training on the
operation and relocation of the proposed MRI system 4. Proof of U.S.
Citizenship or a valid U.S. Immigration form l-151 and an alien
registration card. 5. The proposed MRI operator must furnish a written
statement attesting to be their ability to Read, write and speak the
English language fluently. 6. No prior history of drug/alcohol abuse,
criminal activity, diversion, or mental instability. 7. No prior or
current malpractice proceedings (If the proposed MRI operator has had
or is currently undergoing Malpractice proceedings, information
regarding the outcome of those proceedings is required. 11. The NHGL
will provide required physician support, to be include interpretation
of MRI exams and transcription of results. 12. Staging Site: A. NHGL
will make available an appropriate site for placement of the MRI unit.
B. The contractor will supply telephone service, to include one
hospital line for internal communications, and one dedicated outside
line for remote diagnostic capabilities (if necessary) C. The
contractor will provide all site improvement necessary to be place the
MRI unit operation, to include pad support for the unit, necessary
temporary fencing, and connection to a 420 electrical power line at a
point designated by NHGL. The staging site is concrete with reinforced
rebar and 3 to 4 inches of asphalt. The contractor must provide pad
supports if necessary to prevent the scan system from sinking into the
asphalt. 13. Downtime: The contractor will notify NHGL when downtime
affects scheduled patients. The contractor will make all necessary
alternate scanning arrangements on cases scheduled for that day so
procedures can be performed by other means. These arrangements shall
include, but not be limited to rescheduling of cases for another day of
the same week. 14. Studies repeated for equipment and/or technical
problems will be done at no additional cost to the Government. 15.
Contractor's preventive maintenance schedules (PM) are not to be
arranged during the scheduled hours of operation. 16. Scheduling:The
contractor will perform scanning services at the NHGL two times a week,
scanning is to be to be performed on Tuesday, Wednesday or Thursday
with the first appointment scheduled between 0600 to be 0800 ending
with the completion of the scheduled exams. The maximum number of exams
should not exceed 15 per day. 16.1 The contractor will provide a record
of exams performed after each scheduled day to the Radiology
Department. 16.2 If the regularly scheduled day for the contractor to
perform services falls on a Government holiday, the contractor will
make up that day during the week in which the Government holiday was
on. 17. Workload a. The estimated quantity of patients is a maximum of
15 per exam day. b. Services shall be performed on military and
military beneficiaries 18 Base Support: The contractor and his
employees shall become familiar with and obey all applicable station
regulations. The contractor shall ensure that employees have the
necessary base automobile registrations prior to the start of work. All
motor vehicles operated on this installation by the contractor and his
employees shall be registered with the Security Police according to
applicable installation directives. 19. The contractor will follow the
designated route to the MRI staging site. The map will be provided at
time of award. 20 The following FAR provisions and clauses apply to
this solicitation and are incorporated by reference: FAR 52.212-1

used to evaluate offers: price and past performance, FAR 52.212-3
entitled ''Offeror Representations'' and Certifications-Commercial
Items'', FAR 52,212-4 entitled ''Contract Terms and
Conditions-Commercial Items''Addenda to FAR 52.212-4 : 52.217-9 (a) 30
days (c) 48 months, and FAR 52.212-5 entitled ''Contract Terms and
Conditions Required to Implement Statutes or Executive Orders:
(1),(2),(3),(4),(6),(7), (8),(9) (13),(14),(15). 21. The following
Defense Far Supplement (DFAR) provisions and clauses apply to this
solicitation and are incorporated by reference: DFAR 252.212-7000
entitled Offeror Representations and Certifications-Commercial Items,
and DFAR 252.212-7001 entitled Contract Terms and Conditions Required
to Implement Statutes or Executive Orders Applicable to Defense
Acquisitions of Commercial Items. 22. Past Performance Requirements:
The offeror shall describe past performance on related or similar
contracts held within the last 5 years which are similar in scope,
magnitude, and complexity to that which is detailed in the
solicitation. Offerors who describe similar contracts shall provide a
detailed explanation demonstrating the similarity of the contracts to
the requirements of this solicitation. 22.1 The Government will
consider termination for default, delinquencies, failure to comply with
specifications and or SOW requirements, amount of rework and any other
information which reflects the overall quality of an offeror's past
performance. 22.2 Offerors shall provide the following information
regarding past performance: Contract Number(s), Name and reference
point of contact at federal, state, local Government or commercial
entity for which the contract was performed, Names of sub-contractor(s)
used, if any, and a description of the extent of work performed by
subcontractor(s), and the number, type and severity of any quality,
delivery or cost problems in performing the contract, the corrective
action taken and the effectiveness of the corrective action. 22.3 The
Government reserves the right to obtain information for use in the
evaluation of past performance from any and all sources, including
sources outside the Government. Offerors lacking relevant past
performance history will receive a neutral rating for past performance.
22.4 The proposal of an offeror with no relevant past performance
history, while rated neutral for past performance, may not represent
the most advantageous proposal to the Government and may be an
unsuccessful proposal when compare to the proposals of other offerors.
Offerors must provide the information requested above for past
performance or affirmatively state that they possess no relevant
related or similar past performance. Offerors failing to provide this
past performance information or to assert that it has no relevant
related or similar past performance may be considered ineligible for
award. 22.5 Award will be made to the eligible, responsible offeror
whose offer, conforming to the solicitation, is determined most
advantageous to the Government, price and past performance considered.
The evaluation will consider past performance more important than
price. The Government reserves the right to award the contract to other
than the lowest priced offeror. 23 As a minimum, vendors should provide
this office: a price proposal on letterhead or a SF1449 for the
requested items w/unit price, extended price, past performance
information, prompt payment terms, remittance address, and copies of
FAR 52.212-3 and DFAR 252.212-7000. Responses to this solicitation are
due in the bid room or in the depository located in the bid room at
FISC Norfolk Detachment Philadelphia, Bldg. 2B, 700 Robbins Ave.,
Philadelphia, PA 19111 no later than 4:00 p.m. E.D.T. on 19 Sept 1996.
