commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented additional information included in this notice.
This announcement constitutes the only solicitation, bids are being
requested and a written solicitation will not be issued. Solicitation
number IFB 614-86-96 applies and is issued as an Invitation For Bids
(IFBs). The Standard Industrial Classification Code is 3826 and the
business size is 500 employees. Contractor shall furnish, deliver and
install an ImageNet System ''OR EQUAL'' with printer, review station
and retinal camera to the Dept. of Veterans Affairs Medical Center,
1030 Jefferson Ave., Memphis, TN. This requirement is a fixed price
contract with the following line items: ITEM 1: Quantity 1 ImageNet
System ''OR EQUAL'' to include the following: Pentium Computer System,
SVGA Menu Monitor & 1 Mbyte Controller Card, IMAGEnet H-1024 Imaging
Software, 1280 x 1024 Image Display Board, 1280 x 1024 Image Capture
Board, 1.2 Gbyte Hard Disk Drive, Battery Back UPS, Telephone Modem,
B/W Alignment Monitor (ICG), Alignment Video Camera (ICG), 21'' High
Resolution B/W Image Monitor, Kodak Megaplus Digital Video Camera, PC
Mouse, All cabling for individual Room setting, Support software
packages for diagnostics & maintenance; ITEM 2: Quantity 1 Kodak 8600
Printer to included: SCSI Interface for IMAGEnet Workstation, 1 pack
paper, color ribbon; ITEM 3: Quantity 1 Windows Laser Free Standing
Review Station with CD Drive and Network Card, can enhance and
manipulate images; ITEM 4: Quantity 1, Variable Angle Retinal Camera
with 20, 35 and 50 degree Field Changer, Filter/Flash Programmable
Integration, Tilting Mechanism, 2 Motor Driven 35mm camera bodies,
Internal Fixation Device and 1 Set of filters; ITEM 5: Technical
Training. Delivery of system shall be within 30 days after contract
award. FOB destination to Memphis, TN. Inspection and acceptance will

Evaluation of Commercial Items, the Government intends to make a single
award to the responsible offeror whose offer is the most advantageous
to the Government. Award will be made to the offeror that meets the
synopsis/solicitation's minimum criteria for technical acceptability at
the lowest price. To be determined technically acceptable, the offeror
must furnish product literature that demonstrates the products offered
meet all the requirements stated in the above purchase descriptions.
Note: All offerors are required to submit a completed copy of FAR
Provision 52.212-3, Offeror Representations and
Certifications-Commercial Items with their offer. Failure to submit a
completed copy of this provision with your offer may be cause for
rejection of your offer. Pursuant to FAR Clause 52.212-5, Contract
