Sol:546-27-97 Due:9/24/97, POC Lucille Kaper This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Far Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Solicitation 546-27-97 is issued as a unrestricted request for proposal(RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-40. Standard Industrial Classification Code(SIC) is 7299. Contractor shall provide rental and maintenance of the following equipment(Name Brand or Equal)Pyxis Automatic Controlled Substance Storing and Dispensing System to the VA Medical Center, 1201 NW 16th Street, Miami, Fl. 33125. F.O.B.(Free on Board)destination within the consignee's premises, including installation and deinstallation at the end of the rental period, for the period from the date of award through September 30, 1997. (1)Dispense Unit with Built in Thermal Printer, 12 ea;(2)Pharmacy Work Station, 1 ea;(3)Patient Data System, 1 ea;(4)Auxiliary Unit, 4 ea;(5)Two drawer mini medstation, 1 ea;(6)Maintenance Agreements individually listed on all of the above items. Salient characteristics of the equipment to be furnished are: (a)Must operate similar to an automated teller machine with security password access and will dispense what is asked for by multi-level users(b)Main pharmacy computer workstation must be in direct point-to-point phone line communication to the dispensing unit from the Nursing Unit(c)Pharmacy workstation must be a minimum of 4 M Ram and 200 MB disk or larger(d)Must be a multi-user, multi-tasking, and user freindly system providing mult-level data access(e)Must have a high speed laser or line printer to print reporting information from Pharmacy Host Computer(f)Must have capability to document inventory and to develop automated reports(Batch Reports)that are printed at pre-determined times and dates(g)Must have the ability to interface DHCP for admission, discharge, and transfer information(h)Must be a centralized computer network that interfaces the VA DHCP computer system with a central host computer and each automated dispensing unit(i)Must utilize standard telephone lines(j)Must be able to dispense multiple doses of multiple drugs for a single patient from a single menu option(k)Must be able to assign or delete user access through direct entry into the host computer without the utilization of floppy disc and the host requiring the power to down and rebooting of the system(l)Must provide total control and accountability for each dose dispensed from each unit including name, SSN of patient and name of dispenser(m)Must provide up to the moment dispensing information for each transaction that occurs in each dispensing unit(n)When not in use must automatically remain in a closed and locked position and can only be accessed by authorized personnel. System must be delivered and installed within 30 calendar days after award of contract. Installation and Removal Requirements are: Contractor shall provide all personnel, tools, equipment and materials necessary to install and/or remove the equipment. Training shall be provided at the equipment location with reasonable end-user training at each location of equipment. Service-contractor shall perform service to keep the equipment, and any contractor software or hardware interfaces, contractor software, in, or restore the equipment to good working order based upon the specific needs of the item of equipment at no additional charge, except service due to misappllication, abuse, misues, alteration, or unauthorized repair or instalation. Service shall include adjustments and replacement of parts as necessary to keep the equipment fully operational. If the contractor is unable to repair the equipment, the contractor will replace it. If for any reason the equipment becomes disabled, the contractor shall provide on-site repair by a fully qualified technician within eight working hours. For the purpose of this contract, hours of work for services are defined as seven(7)days per week, 24 hours per day.
