K -- UPGRADE AUTOVALET SYSTEM SOL 578-45-96 DUE 092696 POC Ronald E. Jenkins, Contracting Officer (90C4) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued Solicitation number 578-45-96 is issued as a Request For Proposal (RFP). This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-39. Hines Veterans Affairs Hospital (VAH) intends to purchase an upgrade to Hines VAH owned Automated Garment Distribution Autovalet System, or equal, from Autovalet Systems, LLC. Upgrade is required to transform current Autovalet equipment into state of the art functionality. Upgrade will be required for the following equipment; 1ea.) Autovalet model number MK7/5/700/70/E Uniform Machine No. 7036, 1ea.) Autovalet model number MK7/5/700/70/E Uniform Machine No. 7037, 1ea.) Autovalet model number MK7/5/420/70/E Uniform Machine No. 7038. Upgrade will consist of on site labor to dismantle and remove, as scrap, all existing service kiosk assemblies, all electronic and electrical systems, including wiring loom and cables, and all pick off gantry systems. Existing aluminum storage rail assemblies will be retained for re-use. Contractor shall supply and install, as new, to suit existing storage rails, the following: 3 ea.) Delivery kiosks (one kiosk to be complete with soiled deposit bin and Video 2000 System, or equal), new wiring looms, microprocessor control electronics, or equal; 3ea.) MK7 Pick Off Gantries with microprocessor controls and traveling cables, or equal. Contractor shall be required to test and run each system and upon completion, and acceptance, retrain hospital engineers and system managers on the use and maintenance of new systems. Contractor shall be required to provide a minimum of 12 months warranty on all new parts supplied and insure a minimum of at least ten (10) years spare parts availability. Equivalent equipment upgrade must be compatible and correlatable to the existing Hines VAH owned Autovalet System and provide all necessary hardware and software for upgrade. Delivery is to be F.O.B. destination within 60 days of contract award to Hines VAH Supply Warehouse, Bldg. 220, Hines, IL 60141. Contract award is contingent upon availability of funds with a ninety (90) day acceptance period. The provisions at 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition with no addenda. In accordance with the provisions at 52.212-2, Evaluation--Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be the most advantageous to the Government, price and other factors considered. The following factors, in a descending order of importance, will be used to evaluate offers of equivalent equipment upgrade: (1) Compatibility and correlatability to Hines VAH Autovalet System with the ability to upgrade to a state of the art system (2) Conformation to specifications (3) Price (4) Warranty on upgraded portion of Autovalet System. Offerors are to include a completed copy of the provisions at 52.212-3, Offerors Representations and Certifications-- Commercial Items, with offer. The clause at 52.212-4, Contract Terms and Conditions--Commercial Items, applies to this acquisition with no addenda. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders--Commercial Items, applies to this acquisition with the additional FAR clauses at 52.203-6, Restrictions on Subcontractor Sales to the Government, with Alternate I, 52.203-10, Price or Fee Adjustment for Illegal or Improper Activity, 52.219-8, Utilization of Small Business Concerns and Small Disadvantaged Business Concerns, 2.222-26, Equal Opportunity, 52.222-35 Affirmative Action for Special Disabled and Vietnam Era Veterans, 52.222-36, Affirmative Action for Handicapped Workers and 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era cited in the clause applicable to this acquisition.
